Why Is My Herniated Disc Not Getting Better? | Kearny Mesa, CA
Still Dealing With Herniated Disc Pain After Weeks or Months?
You've tried to give it time.
You've rested.
Maybe you've taken medications, done physical therapy, tried chiropractic treatment, had an injection, changed your workouts, stretched, or followed exercises at home.
And yet you're still dealing with:
-
Lower-back pain
-
Sciatica
-
Buttock or leg pain
-
Numbness or tingling
-
Burning nerve pain
-
Difficulty sitting
-
Difficulty standing or walking
-
Pain with bending or lifting
-
Flare-ups that keep returning
Eventually, the question changes.
Instead of asking:
“How long will this take to heal?”
you start asking:
“Why is my herniated disc NOT getting better?”
If that sounds familiar, it may be time to stop trying one thing after another and find out why your symptoms are continuing.
Sometimes the problem isn't that you haven't tried enough treatments.
Sometimes the problem is that the reason you're still hurting hasn't been clearly identified.
If You're Still in Pain, Find Someone Experienced With Difficult Disc Problems
If your symptoms aren't improving, consider having your condition evaluated by a doctor who regularly works with difficult disc-related problems.
The objective shouldn't simply be:
“What treatment can I try next?”
First, you want to answer:
What is actually causing my symptoms?
Is a spinal nerve still being irritated or compressed?
Does what we're seeing on my MRI match my symptoms?
Is there spinal or foraminal stenosis contributing to the problem?
Is my spine stable?
Is the treatment I've already been receiving appropriate for my condition?
Am I a candidate for Non-Surgical Spinal Decompression?
And equally important:
Is there a reason I should NOT receive spinal decompression?
At The Spinal Decompression & Chiropractic Center of San Diego in Kearny Mesa, determining candidacy comes before recommending treatment.

5 Reasons Your Herniated Disc May Not Be Getting Better
There isn't one explanation that applies to every patient.
But when symptoms continue for weeks or months, there are several important possibilities worth investigating.
1. The Disc May Still Be Irritating a Spinal Nerve
A herniated disc can become particularly problematic when it affects a spinal nerve.
In the lumbar spine, nerve irritation or compression may cause:
-
Sciatica
-
Buttock pain
-
Leg pain
-
Numbness
-
Tingling
-
Burning
-
Weakness
With a cervical herniated disc, symptoms may travel into the shoulder, arm, hand, or fingers.
This distinction matters.
Your local back pain may improve while significant nerve-related symptoms continue.
If you're still experiencing pain, numbness, tingling, or weakness traveling into an arm or leg, that deserves attention.
2. You May Keep Re-Aggravating the Problem
This can create a frustrating cycle.
You start feeling better.
So you do more.
You go back to the gym.
Work in the yard.
Lift something heavy.
Sit at your desk all day.
Take a long flight.
Go golfing.
Then suddenly:
The pain is back.
This doesn't necessarily mean you've completely reinjured your disc.
But it may mean your spine wasn't ready for the amount or type of activity you returned to.
If you're repeatedly going through:
Feel better → increase activity → flare up → recover → flare up again
it may be worth identifying what's continually triggering the problem.
3. The Herniated Disc May Not Be the Entire Problem
This is one reason an MRI report shouldn't be interpreted in isolation.
A patient may have a herniated disc along with other findings that can influence symptoms or treatment decisions.
Depending on the patient, these can include:
-
Degenerative disc changes
-
Foraminal narrowing
-
Spinal stenosis
-
Facet degeneration
-
Alignment changes
-
Spondylolisthesis
-
Spinal instability
-
Other structural or neurological factors
The important question isn't simply:
“Does the MRI show a herniated disc?”
It's:
“Do the imaging findings correlate with your symptoms and examination?”
That distinction can significantly affect what should happen next.
4. You're Getting Temporary Relief—but the Problem Keeps Coming Back
Maybe treatment helps.
But only temporarily.
You feel better for a few hours.
Maybe several days.
Then the symptoms return.
Eventually you find yourself caught in a cycle:
Pain → treatment → temporary relief → pain returns → repeat.
Temporary relief can certainly be valuable when you're hurting.
But if you've been repeating the same approach without meaningful long-term improvement, it may be time to reevaluate the condition instead of simply repeating the treatment.
5. You May Need a Different Treatment Approach
There isn't one treatment that's appropriate for every herniated disc.
Some patients improve with time.
Some respond well to appropriate rehabilitation.
Some benefit from medication or injections.
Some may be candidates for Non-Surgical Spinal Decompression.
And some patients need surgical evaluation.
The important thing is determining which category you fall into.
Physical Therapy Didn't Help My Herniated Disc. Now What?
This is a common reason patients seek another opinion.
If you've already completed weeks of physical therapy but continue experiencing significant symptoms, that doesn't automatically mean physical therapy was wrong.
And it doesn't automatically mean:
“Conservative treatment failed, so now I need surgery.”
Instead, ask:
Why didn't I improve?
Was the nerve still significantly irritated?
Were certain exercises repeatedly aggravating your symptoms?
Was another structural factor involved?
Was the condition appropriate for the exercises being performed?
Could another conservative approach be appropriate?
Those questions can help determine what should happen next.
My Epidural Injection Didn't Work. What Should I Do?
Some patients also seek out the office after receiving an epidural injection.
Injections can be appropriate for certain patients and may help reduce inflammation and pain.
But sometimes the relief is temporary.
And sometimes the patient doesn't experience the improvement they hoped for.
If you're still experiencing significant symptoms after an injection, the next question shouldn't simply be:
“Should I get another injection?”
It may be:
“Why am I still having these symptoms?”
Another evaluation may help clarify what options remain.
Could Non-Surgical Spinal Decompression Be an Option?
For appropriately selected patients, Non-Surgical Spinal Decompression Therapy may be another conservative option for certain cervical and lumbar disc-related conditions.

⭐⭐⭐⭐⭐ From Severe L5 Nerve Pain to Exercising Again
Amanda came to the office after months of severe symptoms and an MRI showing an extruded disc pressing on her L5 nerve.
She had already been scheduled for pain-management injections and a surgical evaluation.
**“Last year, I spent three months constantly going back and forth between doctor's appointments and urgent care. The pain was unbearable—I was taking medication regularly just to get through the day. Getting out of bed was excruciating, and walking or standing felt like torture. Every step filled me with dread.
In December, my doctor finally ordered an MRI. That's when I learned I had an extruded disc pressing on my L5 nerve, which affects the leg and foot. My doctor scheduled two follow-up appointments: one for pain management (spinal injections), and another to evaluate me for surgery.
Then I discovered spinal decompression therapy. I was thrilled to find a non-invasive alternative. I never ended up needing either of those appointments.
Now, I'm walking regularly, standing up straight—something I couldn't do before. When I first sought help, I was so hunched over that I had to use a cane just to move forward. Because of the pain, I hadn't exercised in months. But now, I'm doing a program called 75 Hard, which requires not one but two 45-minute workouts every day, and I've even gotten back to lifting weights!
I'm so thankful I found this place. It's conveniently located near my work, which makes it easy to fit sessions into my lunch break. Dr. Boelk and the staff are incredibly friendly and polite—it's always a pleasure to come in. The environment is so relaxing that I look forward to each visit. I truly thank God for leading me to this alternative. It's made such a difference in my life.”**
— Amanda, San Diego, CA
⭐⭐⭐⭐⭐ “One of the Best Decisions I Made for My Back.”
**“The care I received at Dr. Boelk's office was so wonderful. I went from having to lay down for several hours a day, to being able to live my life normally, and even began working up my exercise capacity. I always felt cared about, and there was a lot of feedback. The exercise and information Dr. Boelk provides is so critical to maintaining positive results and pain reduction with increased mobility and freedom.
All the staff are so caring, and it's one of the best decisions I made for my back!”**
— Kaitelyn H., San Diego, CA
⭐⭐⭐⭐⭐ Not Everyone Who Comes In Is Accepted for Spinal Decompression
Jeff traveled from Tucson looking for another option for his back condition.
After evaluating his condition, Dr. Boelk and Jeff determined that decompression wasn't the appropriate option.
“I found this terrific practice on social media. I followed up and did my own research so I decided to pay them a visit. The place is clean, well organized and run by a very efficient receptionist. I met with the doctor and after a honest and forthright conversation, we both determined that surgery was my only option to correct my back issues. I will return as a customer when the time is right.”
— Jeff J., Tucson, AZ
That's an important part of the evaluation process.
The goal isn't to put everyone on a DRX9000.
The goal is to determine what's causing your symptoms and whether you're actually an appropriate candidate for Non-Surgical Spinal Decompression.
If you're not, you should know that too.
When Is It Time to Stop Waiting?
You don't necessarily need another evaluation every time your back hurts.
But if weeks or months have passed and you're still not making meaningful progress, another evaluation may make sense.
Especially if:
-
Your sciatica keeps returning
-
Pain continues traveling into your leg
-
You have persistent numbness or tingling
-
Sitting is still difficult
-
Walking or standing remains limited
-
You're unable to exercise or work normally
-
Physical therapy hasn't produced the improvement you expected
-
An injection provided little or only temporary relief
-
You've been told surgery may be your next step
-
You're simply tired of not knowing what to do
At that point, you may not need another treatment to try.
You may need a better understanding of why you're not getting better.
