Why Does My Sciatic Pain Keep Coming Back? – San Diego, CA
Sciatica Relief Without Surgery
“I have had a wonderful experience with Dr. Boelk and his staff at The Spinal Decompression & Chiropractic Center of San Diego. They have a great protocol and the best decompression machines. I’ve been to other places with sub par machines and didn’t get any benefit. Dr. Boelk is very attentive and cares about each person, you can see it in how he interacts with everyone. Been undergoing treatment for about 12 sessions now and the sciatica is gone and I’m almost back to normal, didn’t think it would ever heal but even after the first session I had tremendous relief and could sleep mostly pain free for the first time in months. Definitely worth the investment! Thank you!”

Why Does Sciatica Keep Coming Back?
One of the biggest misconceptions about sciatica is assuming the problem is “fixed” simply because symptoms temporarily calm down.
In MANY cases, recurring sciatica involves ongoing mechanical irritation that continues aggravating sensitive discs and nerves repeatedly.
Below are 3 of the MOST common reasons we see recurring sciatica continue to flare up.
1. Daily Compression Continues Irritating the Area
Many patients unknowingly continue compressing the irritated area every single day.
This commonly occurs from:
- prolonged sitting
- long drives
- bending forward repeatedly
- poor posture
- sedentary work
- improper lifting mechanics
Spinal discs naturally absorb compression throughout the day.
When discs are already irritated or injured, repeated compression may continue aggravating sensitive nerves and tissues repeatedly.
This is one reason many people notice:
- worse pain while sitting
- increased symptoms during driving
- more pain later in the day
- flare-ups after travel
- difficulty getting comfortable at night
For SOME patients, the irritated tissue never fully calms down because the mechanical compression stress continues daily.
“There is not enough room to explain the relief Dr. Boelk has brought to me and my entire family. I found him after a year of sciatic pain. Couldn't play the sports I loved, no hockey, golf or surfing. The pain shooting from my hip to my left foot was to the point of unbearable. I had been visiting chiropractors since I was 18, temporary relief, but nothing consistent. Acupuncture helped a bit, physical therapy was unmanageable. I couldn't get a solid night sleep and didn't care to go on. But Then I met the Dr! I tell people regularly, he saved my life.
It was a regular routine of adjustments and exercises. A change of life patterns but 110% worth every effort.
I have since had my wife, daughter, mother, father and multiple friends and clients visit Dr. Boelk. All thanking me for the referral.
So without a doubt, if you are on the verge of surgery, don't do it until you give this Dr a chance to examine you 1st. He is honest, he will tell you if you are beyond repair.
I trust him with my life and my back. So should you!”

2. Aggressive Stretching May Be Irritating the Nerve
One of the most common mistakes we see is aggressive stretching of an already irritated nerve.
Many people are repeatedly told to:
- aggressively stretch hamstrings
- force toe touches
- perform twisting stretches
- do deep yoga movements
…but in SOME cases this may actually increase nerve irritation.
⚠️ Warning signs this may be happening:
- symptoms travel further DOWN the leg
- tingling increases afterward
- numbness worsens
- burning pain intensifies
- symptoms linger longer after stretching
This is why proper evaluation matters.
Not all sciatica behaves the same way.
Different patients respond differently depending on:
- disc involvement
- nerve sensitivity
- inflammation
- spinal mechanics
- stability
- movement compensation patterns
3. The Pain Improved… But the Underlying Problem Did Not
This is one of the BIGGEST reasons recurring sciatica keeps returning.
Temporary pain relief does NOT always mean the underlying problem has been corrected.
Recurring sciatica often involves:
- disc irritation
- disc bulges
- disc herniations
- spinal instability
- poor spinal mechanics
- weak hip and core stability
- repeated compression stress
This is why symptoms frequently return after:
- medications
- injections
- rest alone
- generalized traction
- passive therapies only
At our office, we explain to patients that while Non-Surgical Spinal Decompression (NSSD) and the DRX9000 are often extremely effective, they are still considered passive therapies.
Long-term improvement often requires addressing:
- movement patterns
- spinal mechanics
- posture
- hip mobility
- stability
- repeated daily compression habits
This is one reason our office combines spinal decompression with active rehabilitation strategies whenever appropriate.
“When I first came here I was suffering from sciatica pain for several months non stop. I tried many different non invasive treatments, pain killers but relief was just temporary. I heard about Dr. Boelk on social media and after reading other people testimonials about him I decided to try. It felt better since my first session. After 12 I felt I was improving a lot but still had room to get better so we decided trying 12 more. After 36 sessions I felt I was back to normal. I could get back to surf, jiu jitsu and work after 6 months away. Doctor helped me through the whole process, leaving me free to decide the next step and being honest about expectations. I totally recommend for all my friends that suffer from sciatica pain. It’s worth 100%.”

How Non-Surgical Spinal Decompression (NSSD) May Help
At our office, we exclusively use the DRX9000 True Spinal Decompression System.
Unlike generalized traction devices, true spinal decompression attempts to create a controlled decompressive force targeting specific spinal levels while minimizing muscle guarding responses.
Research involving spinal decompression has been associated with:
- reduced disc pressure
- improved nutrient diffusion
- decreased nerve irritation
- improved spinal mobility
- improved sitting tolerance
However, NOT all spinal decompression treatment is the same.
Proper treatment depends heavily on:
- patient selection
- disc level targeting
- pull angles
- force settings
- positioning
- progression
- movement integration
- stabilization work
This is why patients regularly travel from:
- Arizona
- Nevada
- Oregon
- Utah
- Alaska
- Mexico
- Spain
- The United Kingdom
…to seek Dr. Boelk’s expertise.
Dr. Boelk also trains doctors nationwide through the Spinal Decompression Training Academy on proper DRX9000 protocols and decompression integration.
“I highly recommend Dr. Cassidy Boelk, DC and the team at The Spinal Decompression and Chiropractic Center. I came in with severe sciatica and a herniated disc, and his treatments have made a world of difference. I’m grateful for the care I received and the significant improvement in my quality of life.”

Warning Signs You Should Get Evaluated
You should consider a professional evaluation if you experience:
- worsening leg weakness
- increasing numbness
- pain below the knee
- severe sitting intolerance
- recurring flare-ups
- pain with coughing or sneezing
- symptoms lasting weeks or months
- repeated episodes despite treatment attempts
Not all patients are candidates for Non-Surgical Spinal Decompression.
That is why every patient is evaluated individually before treatment recommendations are made.
