Why Does My Neck Pain Go Down My Arm? | Scripps Ranch, CA
Neck Pain With Arm Pain, Numbness or Tingling? The Problem May Actually Be in Your Neck
It may start with what feels like a simple stiff neck.
Then you notice pain around your shoulder or shoulder blade.
Before long, the pain starts traveling down your arm.
Maybe your hand feels numb.
Your fingers start tingling.
You notice burning, pins and needles, or even weakness in your arm or grip.
Because the symptoms are happening in your shoulder, arm, or hand, it's natural to assume that's where the problem is.
But sometimes the symptoms are in your arm while the source is actually in your neck.
When a nerve in the cervical spine becomes irritated or compressed, symptoms can travel into the shoulder, arm, hand, or fingers. This is commonly referred to as cervical radiculopathy, or what many people call a pinched nerve in the neck.
If your neck pain is traveling down your arm—especially if you're also experiencing numbness, tingling, burning, or weakness—it's worth finding out why.
Neck Pain Going Down Your Arm? Find Out What's Causing It.
If you've been waiting for your symptoms to go away and they aren't improving, don't simply keep guessing.
And don't automatically assume surgery is your next step.
First, find out what's causing your symptoms.
At The Spinal Decompression & Chiropractic Center of San Diego, Dr. Cassidy James Boelk, DC evaluates patients with difficult cervical disc and nerve-related conditions to determine what may be contributing to their symptoms and whether they're an appropriate candidate for Non-Surgical Cervical Spinal Decompression.

Why Can a Problem in Your Neck Cause Pain in Your Arm?
Nerves exit your cervical spine and travel into your shoulders, arms, hands, and fingers.
When one of these nerve roots becomes irritated or compressed, you may experience symptoms farther away from the neck itself.
These can include:
- Neck pain
- Shoulder pain
- Shoulder-blade pain
- Pain traveling down one arm
- Burning
- Numbness
- Tingling
- Pins and needles
- Hand or finger symptoms
- Arm weakness
- Hand or grip weakness
Some patients have substantial neck pain.
Others don't.
You can actually have relatively mild neck discomfort while experiencing significant symptoms farther down your arm.
Where you feel the symptoms isn't always where the problem originates.

Could a Herniated Disc in My Neck Cause Arm Pain?
Yes.
A cervical disc herniation is one possible cause of pain, numbness, or tingling traveling into an arm.
When a cervical disc bulges, protrudes, or herniates, it may irritate or compress a nearby nerve root.
Symptoms may travel along a pattern such as:
Neck → shoulder → shoulder blade → arm → forearm → hand → fingers
But a herniated disc isn't the only possible cause.
Other conditions may include:
- Degenerative disc changes
- Foraminal narrowing
- Bone spurs
- Cervical spondylosis
- Cervical spinal stenosis
- Other structural or neurological causes of nerve irritation
That's why seeing “disc bulge” or “disc herniation” on an MRI doesn't automatically answer everything.
The better question is:
“Does what we're seeing on my MRI correlate with my symptoms and examination?”
Is It My Shoulder or a Pinched Nerve in My Neck?
This is one of the most important distinctions to make.
Shoulder conditions can absolutely cause shoulder and upper-arm pain.
But cervical nerve involvement becomes more important to consider when you also experience:
- Pain traveling farther down the arm
- Numbness
- Tingling
- Burning
- Symptoms into the hand or fingers
- Weakness
An evaluation may include checking:
- Cervical range of motion
- Where your symptoms travel
- Sensation
- Muscle strength
- Reflexes
- Orthopedic findings
- Neurological findings
- Existing imaging when available
The objective isn't simply to give your symptoms a name.
It's to determine where they're coming from and what may be causing them.
Why Does My Arm Hurt More Than My Neck?
This surprises many patients.
If the problem is in your neck, shouldn't your neck hurt the most?
Not necessarily.
Some people experience relatively mild neck discomfort but significant:
- Shoulder-blade pain
- Arm pain
- Forearm pain
- Hand pain
- Finger numbness
- Tingling
- Burning
- Weakness
That's one reason patients can spend weeks treating what they believe is a shoulder problem when a cervical nerve may actually be involved.
The symptoms may be in your arm while the source is in your neck.

What If Physical Therapy Isn't Helping My Neck and Arm Pain?
Physical therapy can be very helpful for many cervical conditions.
But if you've already completed treatment and your symptoms are continuing, it may be time to ask:
“Why am I not getting better?”
Questions that may need to be considered include:
- Is a cervical nerve still irritated or compressed?
- Does the diagnosis match the symptoms?
- Is a disc bulge or herniation contributing?
- Is foraminal narrowing involved?
- Are certain movements repeatedly aggravating the nerve?
- Is another condition contributing?
- Is the cervical spine stable?
- Would another conservative approach be appropriate?
Not improving with one form of conservative treatment doesn't automatically mean every nonsurgical option has failed.
But persistent symptoms deserve another look.
Could Non-Surgical Cervical Spinal Decompression Be an Option?
For appropriately selected patients with certain cervical disc-related conditions, Non-Surgical Cervical Spinal Decompression may be another conservative treatment option.
At The Spinal Decompression & Chiropractic Center of San Diego, Dr. Cassidy James Boelk, DC uses the DRX9000® Non-Surgical Spinal Decompression System.
The practice has 7 DRX9000 Spinal Decompression Systems, including dedicated cervical decompression systems.
But this is important:
Not everyone with neck pain or a cervical disc problem should receive spinal decompression.
Patient selection matters.
The goal is to determine what's causing your symptoms, whether your condition is appropriate for cervical decompression, and whether there are factors that make another approach more appropriate.
