What Causes Neck Pain to Travel Into the Arm?

Your cervical spine contains nerve roots that eventually supply different areas of your shoulders, arms, hands, and fingers.

When one of these nerve roots becomes irritated or compressed, you may experience symptoms away from the neck itself.

These may include:

  • Neck pain
  • Shoulder pain
  • Pain between or around the shoulder blades
  • Pain traveling down one arm
  • Burning pain
  • Numbness
  • Tingling
  • Pins and needles
  • Hand or finger symptoms
  • Arm weakness
  • Hand or grip weakness

Some people have significant neck pain.

Others don't.

In fact, you can have relatively minor neck discomfort while experiencing substantial symptoms farther down the arm.

Where you feel the symptoms isn't always where the problem originates.

Could a Herniated Disc in Your Neck Cause Arm Pain?

Yes.

A cervical disc herniation is one possible cause of symptoms traveling into an arm.

The discs are located between the vertebrae in your neck.

When a cervical disc bulges, protrudes, or herniates, it may irritate or compress a nearby nerve root.

Symptoms may follow a pattern such as:

Neck → shoulder → shoulder blade → arm → forearm → hand → fingers

Depending on the nerve involved, symptoms can include pain, numbness, tingling, burning, or weakness.

But this is important:

A disc bulge on an MRI doesn't automatically mean it's causing your symptoms.

Other conditions may also contribute to cervical nerve irritation, including:

  • Degenerative disc changes
  • Foraminal narrowing
  • Bone spurs
  • Cervical spondylosis
  • Cervical spinal stenosis
  • Other structural or neurological problems

That's why an MRI report shouldn't be evaluated in isolation.

The better question is:

“Does what we're seeing on my MRI correlate with my symptoms and examination?”


Is My Arm Pain Coming From My Neck or My Shoulder?

This is one of the most common questions patients have.

A shoulder condition can certainly cause shoulder and upper-arm pain.

But when symptoms include:

Pain traveling farther down the arm

Numbness

Tingling

Burning

Symptoms into the hand or fingers

Weakness

...cervical nerve involvement becomes an important consideration.

An appropriate evaluation may include checking:

  • Cervical range of motion
  • Where the symptoms travel
  • Sensation
  • Muscle strength
  • Reflexes
  • Orthopedic findings
  • Neurological findings
  • Existing imaging when available

The objective is to determine whether the symptoms appear to originate from the cervical spine and what may be irritating the nerve.


Why Is My Hand or Arm Numb?

Numbness and tingling can be particularly concerning.

An irritated or compressed cervical nerve may produce sensory changes extending into the arm, hand, or fingers.

But not all numbness comes from your neck.

That's important.

There are other conditions that can produce similar symptoms.

That's why looking at a dermatome chart online and trying to diagnose yourself based on which finger is numb isn't enough.

Your:

Symptoms + examination + neurological findings + imaging

should make sense together.


Why Does My Arm Hurt More Than My Neck?

This can seem confusing.

If the problem is in your neck, shouldn't your neck hurt the most?

Not necessarily.

Some patients experience relatively minor neck discomfort but substantial:

  • Shoulder-blade pain
  • Arm pain
  • Forearm pain
  • Hand pain
  • Finger numbness
  • Tingling
  • Burning
  • Weakness

This can cause patients to spend weeks treating what they believe is a shoulder or arm problem.

But if a cervical nerve is involved:

The symptoms may be in your arm while the source is in your neck.

What If Physical Therapy Isn't Helping My Neck and Arm Pain?

Physical therapy can be helpful for many cervical conditions.

“Should I just get another injection?”

When Should Neck and Arm Symptoms Be Taken More Seriously?
