Why Do I Have Leg Pain but No Back Pain? | San Diego, CA

Can you have aching, shooting, burning or stabbing pain down your leg with absolutely no lower-back pain and still have the problem coming from a disc in your back?

Yes. Absolutely.

Can those same symptoms come from something that has nothing to do with a disc?

Also yes.

That's an important distinction because having pain that feels like sciatica doesn't automatically mean you have a herniated disc. But the opposite is also true: having no lower-back pain does not rule out a disc problem.

Think About a Kink in a Garden Hose

One way Dr. Boelk explains this to patients is with a garden hose.

Imagine there's a kink in the hose near the connection point.

You wouldn't necessarily expect the biggest effect to occur right where the kink is.

You'd notice what's happening farther down the hose.

Less water.

Less flow.

Less pressure.

A nerve obviously isn't a garden hose, but the analogy helps explain why the place where you feel a symptom isn't necessarily the place where the problem originates.

A disc in the lower back can bulge or herniate and irritate or compress a nearby nerve root. That nerve then travels away from the spine and contributes to the pathways going into the buttock, leg and foot.

The patient may feel the consequences farther down that pathway.

That's why someone can have a significant disc problem and say, “My back doesn't hurt.”

The back doesn't have to hurt.

Sometimes It's Only the Glute. Sometimes It's Only the Foot.

This is where patients can get thrown off.

They expect a spinal problem to hurt in the spine.

But Dr. Boelk sees patients whose symptoms are isolated almost entirely to one area.

It may be a deep ache in one glute.

It may be burning along the outside of the calf.

It may be numbness across part of the foot.

It may be one toe that feels strange.

It may be a shooting pain that starts in the buttock and runs down the leg.

And sometimes there isn't much pain at all. The main complaint is tingling, numbness or weakness.

Those symptoms can be related to irritation of a lumbar nerve root.

They can also come from other problems.

That's why Dr. Boelk doesn't diagnose a herniated disc simply because someone points to their leg and says they have sciatica.

Leg Pain Doesn't Automatically Mean a Herniated Disc

This part is important.

Not every pain running down the leg comes from a disc.

There are other possible sources of buttock, thigh, calf and foot symptoms. Hip problems, muscular injuries, peripheral nerve irritation and other conditions can sometimes produce symptoms that overlap with what patients call sciatica.

That's why the history matters so much.

Dr. Boelk wants to know where the symptoms start and where they stop.

He wants to know what happens when you sit, stand, walk, bend or exercise.

He wants to know whether coughing or sneezing changes the pain.

He wants to know whether the leg feels weak or whether part of the foot has become numb.

He also wants to know what you've already tried.

If you've spent three months treating your hamstring because it feels tight and it never really changes, that's useful information.

If stretching the hamstring actually causes burning or tingling to travel farther down the leg, that's useful information too.

The goal is to work backward toward the source instead of automatically treating the place that hurts.

If You Have an MRI, Bring the Actual Images

Dr. Boelk prefers to look at the actual MRI images along with the radiologist's report.

Why?

Because MRI reports frequently contain several findings.

A patient may have degeneration at one level, a disc bulge at another and a herniation somewhere else.

Not everything listed on the report is necessarily responsible for the patient's leg pain.

Dr. Boelk wants to see whether the imaging actually fits the clinical picture.

If your symptoms are all in the left leg, is there something on the MRI that could reasonably affect the appropriate nerve on the left?

If the foot is numb, does that make sense with the suspected nerve?

If you've developed weakness, is there a finding that could help explain it?

An MRI is extremely useful when it's needed, but the MRI and the patient have to be looked at together.

Don't Judge the Problem Only by How Much It Hurts

This is particularly important when a nerve may be involved.

A patient may rate the pain only a 3 out of 10 but have persistent numbness in the foot.

Another says the pain has actually improved but one leg is getting weaker.

Dr. Boelk wants to know that.

Tell your healthcare provider if you're having trouble lifting the front of the foot, pushing off normally, walking on your heels or toes, or if the foot begins slapping the ground when you walk.

Increasing numbness matters too.

Most cases of leg pain and sciatica are not emergencies. Rapidly progressing weakness, new bowel or bladder dysfunction, difficulty urinating, or numbness around the groin or saddle area requires prompt medical evaluation rather than waiting for a routine spinal decompression consultation.

Could Non-Surgical Spinal Decompression Be an Option?

Possibly.

But leg pain by itself does not make someone a candidate.

Dr. Boelk evaluates patients with lumbar disc bulges and herniations, degenerative disc disease, disc-related sciatica, foraminal narrowing, selected cases of spinal stenosis and other disc-related problems.

Before recommending treatment, Dr. Boelk looks at the patient's symptoms, history, neurological findings, imaging when available, spinal stability, bone health, previous surgery and overall medical history.

There are also situations where decompression may not be appropriate, including significant osteoporosis, an unhealed fracture, unstable spondylolisthesis, abdominal aortic aneurysm, pregnancy for lumbar decompression, certain malignancies and other medical or structural conditions.

Previous spinal surgery has to be considered carefully, especially when there is fusion or metal hardware at the level that would need treatment.
