SCIATICA TREATMENT POWAY, CA
Why Does My Sciatica Keep Coming Back?
If you're suffering from sciatica, you've probably asked yourself the same question over and over:
"Why does it keep coming back?"
Maybe your pain improved for a few days or a few weeks after physical therapy, chiropractic care, massage, or medication. Then, without warning, it returned.
Perhaps it's worse when you're sitting in your car, getting out of bed in the morning, or standing for long periods. Maybe the pain travels from your lower back into your buttock, down the back of your leg, and sometimes all the way into your foot.
If that sounds familiar, you're not alone.
Many patients from Poway come to The Spinal Decompression & Chiropractic Center of San Diego feeling frustrated because they've tried multiple treatments without lasting relief. Some have even been told their only remaining option may be injections or surgery.
While surgery is absolutely the right choice for some people, many cases of sciatica can be evaluated and managed conservatively. The first step is understanding why the pain keeps returning instead of simply treating the symptoms.

"Dr. Boelk has been a lifesaver. I suffered from constant low back pain and sciatica for more than eight months. Pain medications didn't help, and I was headed toward multiple injections in hopes of finding relief. After beginning treatment with Dr. Boelk, everything changed. Today, my pain is completely gone. As a firefighter who depends on his body every day to help others, being pain-free has made a tremendous difference in both my career and quality of life."

WHAT IS SCIATICA?
Many people think sciatica is a diagnosis.
It isn't.
Sciatica is a symptom.
The sciatic nerve is the largest nerve in your body. It begins in your lower back, travels through your buttock, down the back of your leg, and branches into your foot.
When that nerve becomes irritated or compressed, symptoms may include:
✔ Pain traveling down the leg
✔ Burning sensations
✔ Tingling
✔ Numbness
✔ Muscle weakness
✔ Pain while sitting
✔ Difficulty walking
The important question isn't simply, "Do I have sciatica?"
The real question is:
"Why is my sciatic nerve irritated?"
WHAT CAUSES SCIATICA?
Several different conditions may cause sciatic nerve irritation, including:
- Herniated discs
- Bulging discs
- Disc protrusions
- Disc extrusions
- Foraminal stenosis
- Degenerative disc disease
- Spinal stenosis
- Less commonly, conditions involving the piriformis muscle or other structures around the nerve
This is why obtaining an accurate diagnosis matters so much.
Two people may both have "sciatica," but the underlying cause—and therefore the most appropriate treatment—may be completely different.
WHY DOES SCIATICA KEEP COMING BACK?
This is one of the most common questions I hear from patients.
Often, the reason isn't that previous treatment "failed." It's that the treatment may have reduced inflammation or muscle spasm without fully addressing the underlying cause of the nerve irritation.
For example, if a disc injury continues to place mechanical stress on a spinal nerve, symptoms may temporarily improve and then return as your daily activities place stress on that area again.
Another reason is returning to normal activities too quickly without restoring spinal stability, strength, and movement patterns.
Recovery is rarely just about reducing pain. It's also about improving the way your spine functions so the irritated tissues have an opportunity to calm down and heal.
WHY DOES SITTING MAKE SCIATICA WORSE?
One thing many patients notice is that walking feels better than sitting.
Why?
When you sit, the pressure inside the lumbar discs generally increases compared with standing. For some people with certain disc-related conditions, prolonged sitting may aggravate symptoms.
That doesn't mean sitting is "bad."
It simply means that changing positions regularly and avoiding prolonged static postures may help reduce irritation throughout the day.
Small changes can sometimes make a meaningful difference.
HOW DOES NON-SURGICAL SPINAL DECOMPRESSION FIT INTO TREATMENT?
Depending on the cause of your sciatica, treatment options may include:
- Activity modification
- Physical therapy
- Exercise
- Chiropractic care
- Rehabilitation
- Medications
- Epidural injections
- Non-Surgical Spinal Decompression
- Surgery, when medically appropriate
At The Spinal Decompression & Chiropractic Center of San Diego, Dr. Cassidy James Boelk, DC uses Non-Surgical Spinal Decompression as one treatment option for appropriately selected patients with certain disc-related conditions.
Treatment is individualized. Not every patient is a candidate, which is why every recommendation begins with a thorough history, examination, and review of any MRI studies you already have.
SOMETHING YOU CAN TRY TODAY
If sitting increases your sciatica:
✔ Stand and walk for two or three minutes every 20 to 30 minutes.
✔ Place a small lumbar support behind your lower back while driving.
✔ Avoid spending long periods slouched on a soft couch.
These suggestions won't cure sciatica, but many patients find they help reduce irritation while waiting for an evaluation.

WHY EXPERIENCE MATTERS WHEN TREATING SCIATICA
Sciatica isn't a diagnosis—it's a symptom.
Finding the true cause of sciatic nerve pain is one of the most important parts of developing the right treatment plan.
Although many cases of sciatica are caused by a herniated disc or other disc-related condition, they can also result from spinal stenosis, foraminal stenosis, degenerative disc disease, instability, or other conditions affecting the lower back.
Because treatment depends on the underlying cause, experience matters.
For more than 22 years, Dr. Cassidy James Boelk, DC has focused on evaluating and treating patients suffering from disc-related spinal conditions. During that time, he has personally performed more than 100,000 Non-Surgical Spinal Decompression treatments and has helped thousands of patients understand their diagnosis and explore conservative treatment options before considering surgery, when appropriate.
Rather than following a one-size-fits-all approach, every patient receives an individualized evaluation based on their symptoms, physical examination, imaging studies, and overall health.

HOW DO I CHOOSE THE RIGHT SCIATICA DOCTOR?
If you've been dealing with sciatica for weeks or months, choosing the right healthcare provider can feel overwhelming. Every office claims to have the best treatment, but asking the right questions can help you make a more informed decision.
Does The Doctor Regularly Treat Sciatica?
Sciatica is one of the most common conditions affecting the lower back and legs, but it can have many different causes.
Choose a provider who routinely evaluates patients with herniated discs, bulging discs, spinal stenosis, foraminal stenosis, and other conditions that commonly cause sciatic nerve pain.
Will The Doctor Personally Review My MRI?
Your MRI provides valuable information, but it should never be interpreted in isolation.
A thorough evaluation compares your MRI findings with your symptoms and physical examination to determine whether the imaging actually explains your pain.
Does The Office Focus On Long-Term Recovery?
Reducing pain is important.
Helping reduce the likelihood of future flare-ups is equally important.
Look for an office that discusses rehabilitation, spinal stabilization, posture, flexibility, and strategies to help protect your spine after your symptoms improve.
Will They Tell Me If I'm NOT A Candidate?
This may be the most important question you ask.
Not everyone with sciatica is a candidate for Non-Surgical Spinal Decompression.
Some patients require surgery.
Others may benefit from physical therapy, injections, medications, or other conservative approaches.
An honest provider should explain why they're recommending a treatment—and just as importantly, why they may not recommend it.
