My Surgeon Says I Need Neck Surgery. Should I Get a Second Opinion? – Kearny Mesa, CA

What Patients in Kearny Mesa Should Know Before Making One of the Biggest Healthcare Decisions of Their Lives

If you've recently been told you need neck surgery, you're probably wondering whether it's really the only option.

Maybe your MRI showed a herniated cervical disc, spinal stenosis, or a pinched nerve in your neck. Perhaps you're experiencing neck pain, pain radiating into your shoulder or arm, numbness or tingling in your hand, or weakness that's making everyday activities more difficult. After meeting with a spine surgeon, you've been told surgery may be the next step.

Before making that decision, it's completely reasonable to ask one important question:

Should I get a second opinion?

Every week, we meet patients from Kearny Mesa and throughout San Diego who are asking that exact question. Some ultimately move forward with surgery. Others discover they still have conservative treatment options worth considering. Regardless of the outcome, almost everyone leaves feeling more confident because they better understand their diagnosis and why a particular treatment was recommended.

Should You Get a Second Opinion?

In many cases, the answer is yes.

Getting a second opinion doesn't mean your surgeon is wrong, and it doesn't mean surgery isn't necessary. It simply gives you another opportunity to fully understand your diagnosis, review your MRI, discuss your symptoms, and make sure all of your treatment options have been considered before making such an important decision.

Many patients actually feel more confident after getting a second opinion, regardless of whether the recommendation changes. Sometimes the second doctor agrees surgery is the most appropriate option. Other times, they recommend trying conservative treatment first. Either way, you'll have a better understanding of your condition and greater confidence in your decision.

"I wasn't looking for someone to tell me not to have surgery. I just wanted to know I was making the right decision."

That's something we hear from patients all the time.

What Should a Second Opinion Include?

A second opinion should involve much more than someone looking at your MRI for a few minutes.

A thorough evaluation should include:

  • A review of your symptoms and how they affect your daily life.

  • A complete orthopedic and neurological examination.

  • A careful review of your MRI or other imaging.

  • A discussion of treatments you've already tried.

  • An honest conversation about whether surgery—or conservative care—is the most appropriate next step.

Your MRI is an important part of the evaluation, but it's only one piece of the puzzle. Your symptoms, examination findings, medical history, activity level, and goals should all be considered before deciding whether neck surgery is truly necessary.

Do Spine Surgeons Always Agree?

Not always.

Like every area of medicine, different physicians sometimes recommend different treatment plans based on their experience, training, and interpretation of your condition. One surgeon may recommend surgery sooner, while another may suggest continuing conservative treatment for a little longer.

That doesn't automatically make one opinion right and the other wrong. It simply means there can be more than one reasonable approach depending on the individual patient.

This is one of the biggest reasons many people choose to seek a second opinion before scheduling neck surgery.

What If the Opinions Are Different?

It's not uncommon for two doctors to recommend different treatment plans for the same neck condition.

One physician may recommend surgery sooner, while another believes conservative treatment is still appropriate. That doesn't necessarily mean one doctor is right and the other is wrong. It often means your condition falls into an area where more than one reasonable treatment approach exists.

Instead of asking, "Which doctor is correct?" ask each physician to explain why they're making that recommendation. Once you understand the reasoning behind each option, you'll be in a much better position to make the decision that's right for you.

Does Waiting Mean I'll Make My Neck Worse?

This is another question we hear frequently.

For many cervical spine conditions, taking a week or two to better understand your diagnosis or obtain a second opinion is perfectly reasonable. Most patients have been dealing with neck pain, arm pain, numbness, or tingling for weeks, months, or even years before they ever consult a spine surgeon.

However, there are situations where waiting is not appropriate. If you're experiencing any of the following, you should seek immediate medical attention:

  • Progressive weakness in your arm or hand

  • Loss of coordination that's rapidly getting worse

  • Difficulty walking or maintaining your balance

  • Loss of bowel or bladder control

  • Significant or rapidly worsening neurological symptoms

These situations require immediate medical evaluation and should never be ignored.

What If I've Already Tried Physical Therapy, Injections, or Chiropractic Care?

Many of our patients have.

By the time they visit our office, they've often completed physical therapy, tried chiropractic care, received cervical epidural injections, taken medications, or spent months performing home exercises without finding lasting relief.

That doesn't automatically mean surgery is your only remaining option.

Sometimes the diagnosis needs another look. Sometimes a different conservative treatment approach may be appropriate. And sometimes surgery truly is the best next step.

The important thing is making that decision after a thorough evaluation—not simply because previous treatments didn't provide the results you were hoping for.

We Don't Believe Every Patient Is a Candidate

One thing that makes our office different is that we don't believe every patient is a candidate for spinal decompression or conservative treatment.

Over the past 22 years, we've referred many patients to spine surgeons because we believed surgery was the most appropriate treatment for their condition. We've also cared for thousands of patients who were appropriate candidates for conservative care and were able to avoid surgery.

Our responsibility isn't to convince you to choose one treatment over another.

Our responsibility is to give you an honest evaluation, explain your options, and help you make an informed decision based on your individual condition.

My goal has never been to talk patients out of surgery. Throughout my career, I've referred many patients to spine surgeons when I believed surgery was truly the best option.
