My Surgeon Says I Need Back Surgery. Should I Get a Second Opinion? – Kearny Mesa, CA

What Patients in Kearny Mesa Should Know Before Making One of the Biggest Healthcare Decisions of Their Lives

If you've recently been told you need back surgery, you're probably feeling a mix of emotions. Maybe you're relieved to finally have an answer after months of pain. Maybe you're nervous about having surgery. Or maybe you're wondering if surgery is really your only option.

Every week, we meet patients from Kearny Mesa and throughout San Diego who are asking those same questions. Some have been diagnosed with a herniated disc. Others have spinal stenosis, sciatica, degenerative disc disease, or another spinal condition. Almost all of them have one thing in common—they want to make sure they're making the right decision before moving forward.

That's where a second opinion can be valuable.

Should You Get a Second Opinion?

In many cases, yes.

Getting a second opinion doesn't mean your surgeon is wrong, and it doesn't mean surgery isn't necessary. It simply gives you another opportunity to fully understand your diagnosis, review your MRI, discuss your symptoms, and make sure all of your treatment options have been considered before making such an important decision.

Many patients actually feel more confident after a second opinion, regardless of whether the recommendation changes. Sometimes the second doctor agrees that surgery is the best choice. Other times, they recommend trying conservative treatment first. Either way, you leave with a better understanding of your condition and why a particular treatment has been recommended.

"I wasn't looking for someone to tell me not to have surgery. I just wanted to know I was making the right decision."

That's something we hear from patients all the time.

What Should a Second Opinion Actually Include?

A second opinion should be much more than someone looking at your MRI for a few minutes.

A thorough evaluation should include:

  • A review of your symptoms and how they affect your daily life.

  • A complete physical and neurological examination.

  • A careful review of your MRI or other imaging.

  • A discussion of treatments you've already tried.

  • An honest conversation about whether surgery—or conservative care—is the most appropriate next step.

An MRI is an important part of the evaluation, but it's only one piece of the puzzle. Your symptoms, examination findings, medical history, and goals all matter when deciding how your condition should be treated.

Do Spine Surgeons Always Agree?

Not necessarily.

Just like any other area of medicine, different physicians sometimes recommend different treatment plans based on their experience, training, and interpretation of your condition. One surgeon may recommend surgery sooner, while another may suggest continuing conservative care for a little longer.

That doesn't automatically make one opinion right and the other wrong. It simply means there can be more than one reasonable approach depending on the individual patient.

This is one of the biggest reasons many people choose to seek a second opinion before scheduling surgery.

What If the Opinions Are Different?

It's not uncommon for two doctors to recommend different treatment plans, especially when you're dealing with chronic back pain, sciatica, or a herniated disc.

One physician may believe surgery is the best option, while another recommends continuing with conservative treatment first. That doesn't necessarily mean one doctor is right and the other is wrong. It often means your condition falls into an area where more than one reasonable treatment approach exists.

Instead of asking, "Which doctor is correct?" ask each physician to explain why they're making that recommendation. Understanding the reasoning behind each option will help you make a much more informed decision.

Does Waiting Mean I'll Make My Back Worse?

This is one of the most common concerns we hear.

For many spinal conditions, taking a week or two to better understand your diagnosis or obtain a second opinion is perfectly reasonable. Most patients have been dealing with their symptoms for months—or even years—before they ever meet with a spine specialist.

However, there are situations where waiting is not appropriate. If you're experiencing any of the following, you should seek immediate medical attention:

  • Progressive muscle weakness

  • Loss of bowel or bladder control

  • Numbness in the groin or saddle area

  • Significant or rapidly worsening neurological symptoms

What If I've Already Tried Physical Therapy, Injections, or Chiropractic Care?

Many of our patients have.

By the time they arrive at our office, they've often completed physical therapy, received spinal injections, tried chiropractic care, taken medications, or spent months doing home exercises without lasting relief.

That doesn't automatically mean surgery is your only remaining option.

Sometimes the diagnosis needs another look. Sometimes a different conservative treatment may be appropriate. And sometimes surgery truly is the best next step.

The important thing is making that decision after a thorough evaluation—not simply because previous treatments didn't provide the results you were hoping for.

We Don't Believe Every Patient Is a Candidate

One thing that makes our office different is that we don't believe every patient should undergo spinal decompression.

Over the past 22 years, we've referred many patients to spine surgeons because we believed surgery was the most appropriate treatment for their condition. We've also cared for thousands of patients who were able to improve without surgery because they were appropriate candidates for conservative care.

Our responsibility isn't to convince you to choose one treatment over another.

Our responsibility is to give you an honest evaluation, explain your options, and help you make an informed decision based on your individual condition.

A Personal Message From Dr. Cassidy James Boelk, DC

If you've been told you need back surgery, I understand how overwhelming that conversation can be. It's a major decision, and before moving forward, you deserve to fully understand your diagnosis and all of your treatment options.

My goal has never been to talk patients out of surgery. In fact, throughout my career I've referred many patients to spine surgeons when I believed surgery was the best course of action. I've also cared for many patients who were appropriate candidates for conservative treatment and were able to avoid surgery.

Every patient is different, which is why every patient deserves an honest, individualized evaluation. If you're looking for a second opinion, I'd be happy to review your MRI, perform a thorough examination, answer your questions, and give you my honest opinion. Whether that opinion leads you toward conservative care or surgery, my goal is simply to help you make an informed decision with confidence.
