My MRI Says I Have a Herniated Disc. Do I Need Surgery? – Mira Mesa, CA

What Patients in Mira Mesa Should Know Before Deciding on Back Surgery

If you've recently had an MRI and were told you have a herniated disc, you're probably wondering what comes next.

Do I need surgery?

Can this heal without surgery?

Should I get another opinion before making a decision?

These are some of the most common questions we hear from new patients.

Every week, we meet people from Mira Mesa who come to our office after being told they have a herniated disc, bulging disc, spinal stenosis, or degenerative disc disease. Many have already tried physical therapy, medications, injections, chiropractic care, or simply waiting to see if the pain would improve.

Some have been told surgery may be their next step.

Others simply want to understand whether there are any reasonable non-surgical options before making that decision.

One of the first things we explain is this:

An MRI is one of the best tools we have for looking at your spine—but it doesn't tell the whole story.

An MRI can show us what your spine looks like.

It cannot tell us:

  • Whether the herniated disc is actually causing your pain

  • How severe your symptoms are

  • Whether you're a candidate for conservative treatment

  • Whether surgery is truly necessary

Those answers come from putting all the pieces together.

What Is a Herniated Disc?

Between each vertebra in your spine is a disc that acts as both a cushion and a shock absorber.

A herniated disc occurs when part of that disc pushes beyond its normal boundary. If the disc begins irritating a nearby spinal nerve, it can lead to symptoms such as:

  • Low back pain

  • Sciatica

  • Buttock pain

  • Leg pain

  • Numbness or tingling

  • Muscle weakness

However, not every herniated disc causes symptoms.

Many people have herniated or bulging discs and don't even know it because the disc isn't affecting a nerve.

That's why it's important not to panic after reading your MRI report.

Your MRI Doesn't Always Explain Your Pain

This surprises many patients.

We've treated people with very large herniated discs who had only mild discomfort.

We've also evaluated patients with relatively small disc bulges who could barely sit through dinner, walk around the block, or sleep through the night because of severe sciatica.

The size of the disc isn't always the most important factor.

The location of the herniation, the amount of inflammation around the nerve, and your body's individual response often play a much larger role.

That's why treatment decisions should never be based solely on an MRI.

The Most Important Question Isn't "What Does My MRI Show?"

The better question is:

"Do my MRI findings actually match my symptoms?"

That's where a thorough evaluation becomes so important.

Before recommending any treatment, we consider:

  • Your symptoms

  • Your medical history

  • Your orthopedic examination

  • Your neurological findings

  • Your MRI

  • Your goals and activity level

Only after looking at the complete picture can we determine whether conservative treatment, additional testing, or surgery makes the most sense.

Our philosophy has always been simple:

Treat the patient—not just the MRI.

 

When Is Surgery Actually Necessary?

Finding out you have a herniated disc doesn't automatically mean surgery is your next step.

In fact, many patients improve with the right conservative treatment and never require an operation.

Others, however, may ultimately become appropriate candidates for surgery because of progressive neurological symptoms or persistent pain that hasn't responded to appropriate care.

The key is making the decision based on the entire clinical picture—not just your MRI.

When Should You Consider a Surgical Consultation?

Every patient is different, but surgery may become appropriate if you develop:

  • Progressive muscle weakness

  • Loss of bowel or bladder control

  • Numbness in the groin or saddle region

  • Significant neurological deficits

  • Severe pain that continues despite appropriate conservative treatment

For many patients, however, there are still conservative treatment options worth exploring before making that decision.

Can a Herniated Disc Heal Without Surgery?

In many cases, it can.

Research has shown that some herniated discs gradually shrink over time as the body naturally reabsorbs part of the damaged disc material.

That doesn't happen in every case, and not every patient can avoid surgery.

What it does mean is that seeing "herniated disc" on an MRI doesn't automatically make surgery your only option.

We Don't Believe Every Patient Is a Candidate

One of the biggest differences about our office is that we don't recommend spinal decompression for everyone.

Over the past 22 years, we've evaluated thousands of patients with herniated discs, sciatica, spinal stenosis, degenerative disc disease, and chronic neck and back pain.

Some have been excellent candidates for non-surgical spinal decompression.

Others have not.

If we believe another conservative treatment is more appropriate, we'll explain why.

If we believe surgery offers the best chance for recovery, we'll tell you that too.

Our goal has never been to treat everyone.

Our goal is to recommend the treatment that's most appropriate for your specific condition.
