What Your MRI Really Means—and What It Doesn't

If you've recently had an MRI and were told you have a herniated disc, you're probably asking yourself one question:

Does this mean I need surgery?

For many people, seeing the words herniated disc on an MRI report can be frightening. It's easy to assume that because the MRI found something abnormal, surgery is the next logical step.

Fortunately, that's not always the case.

Every week, we meet patients from Kearny Mesa and throughout San Diego who arrive carrying an MRI report that's filled with medical terminology they don't fully understand. They've read words like disc herniation, disc bulge, disc protrusion, disc extrusion, or spinal stenosis, and naturally they're concerned.

One of the first things we explain is that an MRI is an excellent diagnostic tool—but it doesn't make treatment decisions.

What Is a Herniated Disc?

Between each vertebra in your spine is a disc that acts as a cushion and shock absorber. A herniated disc occurs when part of the disc pushes beyond its normal boundary. Depending on where it's located and whether it's irritating a nearby nerve, it may cause symptoms such as:

  • Low back pain

  • Sciatica

  • Buttock pain

  • Leg pain

  • Numbness or tingling

  • Weakness

However, not every herniated disc causes symptoms.

In fact, many people have disc herniations and never know it because the disc isn't pressing on a nerve or creating significant inflammation.

Can an MRI Tell How Much Pain I'll Have?

No.

This is one of the biggest misconceptions we see.

An MRI can show changes in the spine, but it can't tell us how much pain you're experiencing. It also can't tell us how much those findings are affecting your daily life.

We've seen patients with very large herniated discs who report surprisingly little pain. We've also seen patients with relatively small disc bulges who can barely sit, walk, or sleep because the irritated nerve is causing severe sciatica.

That's why your MRI should always be interpreted together with your symptoms and physical examination.

Why Your Symptoms Matter Just As Much As Your MRI

Your MRI is only one piece of the puzzle.

Before recommending treatment, it's important to consider:

  • Where your pain is located

  • Whether pain travels into your leg

  • Numbness or tingling

  • Muscle weakness

  • Reflex changes

  • How long you've had symptoms

  • Which treatments you've already tried

  • How your condition affects your daily activities

When your symptoms, examination, and MRI findings all point toward the same diagnosis, it's much easier to develop an appropriate treatment plan.

That's why we never recommend treatment based solely on an MRI report.

When Is Surgery Actually Necessary?

One of the biggest misconceptions is that every herniated disc eventually requires surgery.

Fortunately, that isn't true.

Many people with herniated discs improve with time and appropriate conservative treatment. Others continue to have symptoms that significantly affect their quality of life and may ultimately be appropriate candidates for surgery.

Every situation is different, which is why it's so important to evaluate the entire patient—not just the MRI.

What Symptoms May Make Surgery More Appropriate?

While every patient should be evaluated individually, surgery may be recommended when someone has:

  • Progressive muscle weakness

  • Loss of bowel or bladder control

  • Numbness in the groin or saddle area

  • Severe nerve compression causing significant neurological deficits

  • Persistent pain that hasn't responded to appropriate conservative treatment and is significantly affecting daily life

The important thing to remember is that the decision isn't based solely on the MRI. It's based on how your symptoms, examination, and imaging fit together.

Can a Herniated Disc Heal Without Surgery?

In many cases, yes.

Research has shown that some herniated discs can decrease in size over time as the body gradually breaks down and reabsorbs part of the damaged disc material. While every patient is different, this is one reason many physicians recommend conservative treatment before considering surgery when it's appropriate.

That doesn't mean every herniated disc will heal completely, and it certainly doesn't mean conservative care is right for everyone. It simply means surgery isn't automatically required just because your MRI shows a herniated disc.

What If I've Already Tried Physical Therapy, Injections, or Chiropractic Care?

Many of our patients have.

By the time they arrive at our office, they've often completed physical therapy, received spinal injections, tried chiropractic care, taken medications, or spent months performing home exercises without finding lasting relief.

That doesn't automatically mean surgery is the only option remaining.

Sometimes the diagnosis needs another look. Sometimes a different conservative treatment approach is appropriate. And sometimes surgery truly is the best next step.

The goal is making the right decision based on a thorough evaluation—not simply assuming surgery is necessary because previous treatments weren't successful.
