How Do I Know If My Herniated Disc Is Healing? | Mira Mesa, CA
How Can You Tell If Your Herniated Disc Is Actually Improving?
Recovering from a herniated disc can be confusing.
You may have a great weekend and think you're finally getting better—then sit through a long workday or car ride and feel some of the pain return.
Does that mean you're back to where you started?
Not necessarily.
Herniated disc recovery isn't always perfectly linear. Symptoms can fluctuate as inflammation changes, nerve irritation settles, and you gradually increase your activity.
Instead of judging your recovery based on a single good or bad day, look at the overall direction of your symptoms and function over time.
Some of the changes that may indicate improvement include:
-
Less intense pain
-
Pain occurring less frequently
-
Sciatica or arm pain traveling a shorter distance
-
Decreased numbness or tingling
-
Improving muscle strength
-
Sitting longer
-
Standing longer
-
Walking farther
-
Sleeping more comfortably
-
Moving with less restriction
-
Returning to normal activities
One of the simplest ways to measure progress is to ask:
“What can I comfortably do now that I couldn't do a few weeks ago?”
That answer can sometimes tell you more about your recovery than your pain score alone.
⭐⭐⭐⭐⭐ From a Cane and Severe L5 Nerve Pain to Exercising Again
Amanda came to the office after months of severe symptoms.
Her MRI revealed an extruded disc pressing on her L5 nerve. She was struggling to walk and stand upright and had already been scheduled for spinal injections and a surgical evaluation.
Her experience demonstrates how dramatically function can change as a patient improves.
**“Last year, I spent three months constantly going back and forth between doctor's appointments and urgent care. The pain was unbearable—I was taking medication regularly just to get through the day. Getting out of bed was excruciating, and walking or standing felt like torture. Every step filled me with dread.
In December, my doctor finally ordered an MRI. That's when I learned I had an extruded disc pressing on my L5 nerve, which affects the leg and foot. My doctor scheduled two follow-up appointments: one for pain management (spinal injections), and another to evaluate me for surgery.
Then I discovered spinal decompression therapy. I was thrilled to find a non-invasive alternative. I never ended up needing either of those appointments.
Now, I'm walking regularly, standing up straight—something I couldn't do before. When I first sought help, I was so hunched over that I had to use a cane just to move forward. Because of the pain, I hadn't exercised in months. But now, I'm doing a program called 75 Hard, which requires not one but two 45-minute workouts every day, and I've even gotten back to lifting weights!
I'm so thankful I found this place. It's conveniently located near my work, which makes it easy to fit sessions into my lunch break. Dr. Boelk and the staff are incredibly friendly and polite—it's always a pleasure to come in. The environment is so relaxing that I look forward to each visit. I truly thank God for leading me to this alternative. It's made such a difference in my life.”**
— Amanda, San Diego, CA
Amanda's experience is an individual patient result. Results vary, and not every patient will experience the same outcome or avoid injections or surgery.
What stands out is that her progress wasn't measured by pain alone.
She went from struggling to stand upright and needing a cane to walking regularly, exercising and lifting weights again.
Those are meaningful functional changes.

7 Signs Your Herniated Disc May Be Healing
1. You're Having More Good Hours and Good Days
You don't necessarily need to become completely pain-free overnight to be making progress.
Think about how your symptoms behaved when the problem was at its worst.
Maybe pain was constant.
Perhaps every morning was difficult.
Maybe sitting for 20 minutes caused significant symptoms.
If you're now experiencing longer periods where the pain is manageable—or isn't constantly demanding your attention—that may represent improvement.
Look for the trend over several weeks, not several hours.
2. Your Sciatica or Arm Pain Doesn't Travel as Far
Where you're experiencing symptoms can matter.
With a lumbar disc problem, pain may initially travel from the lower back or buttock all the way into the calf, ankle, or foot.
With a cervical disc problem, symptoms may extend through the shoulder and arm into the hand or fingers.
As the condition improves, some patients notice that these symptoms don't travel as far.
For example, leg pain that previously extended into the foot may become limited primarily to the thigh or buttock.
This can be an encouraging change even if some discomfort remains.
3. Numbness, Tingling or Burning Is Decreasing
Disc-related nerve irritation doesn't always present as pain.
It can also produce:
-
Numbness
-
Tingling
-
Burning
-
Pins and needles
-
Altered sensation
Neurological symptoms can sometimes take longer to improve than pain.
If these sensations are becoming less frequent, less intense, or affecting a smaller area, that may be a positive sign.
4. Muscle Strength Is Improving
When a herniated disc affects a spinal nerve, weakness can sometimes develop.
Lumbar nerve involvement may affect the leg, ankle, or foot.
Cervical nerve involvement may affect the shoulder, arm, wrist, hand, or fingers.
Improving strength may indicate neurological improvement.
However, increasing or rapidly progressing weakness isn't something to simply wait out.
It deserves appropriate medical evaluation.
5. You Can Sit, Stand or Walk Longer
This is one of the most useful ways to monitor progress.
Suppose several weeks ago you could only sit for 15 minutes before your leg began hurting.
Now you can comfortably sit for 45 minutes.
Or perhaps you previously couldn't walk around the block and now you're walking for 20 or 30 minutes.
Those changes matter.
Improved tolerance for everyday activities may demonstrate progress even if you're not completely symptom-free.
6. You're Sleeping More Comfortably
Sleep is often overlooked when evaluating recovery.
Significant disc and nerve pain can make it difficult to fall asleep, stay asleep, roll over, or find a comfortable position.
If you're waking less frequently because of pain or sleeping for longer periods, that's another functional improvement worth noting.
7. You're Doing More of the Things You Normally Do
This may ultimately be the most important measurement.
Ask yourself:
“Is my world getting bigger again?”
Can you:
-
Work more comfortably?
-
Drive farther?
-
Exercise?
-
Walk?
-
Travel?
-
Go out with your family?
-
Return to recreational activities?
-
Get through a normal day without constantly planning around your back?
Those changes are meaningful.
The objective isn't simply to make a pain score lower.
It's to help you return to your life.
Can You Have Pain and Still Be Healing?
Yes.
Healing doesn't necessarily mean the complete absence of symptoms.
Inflammation and nerve irritation can fluctuate.
As patients begin doing more, they may temporarily experience soreness or an increase in symptoms.
The better questions are:
-
Are flare-ups less severe?
-
Are they happening less frequently?
-
Do they resolve faster?
-
Are you able to do more before symptoms appear?
-
Is your strength improving?
-
Are neurological symptoms decreasing?
A temporary flare-up and a progressively worsening neurological condition are not the same thing.
Why Does Sciatica Sometimes Take Longer to Improve Than Back Pain?
When a spinal nerve has been significantly irritated or compressed, nerve-related symptoms can sometimes persist even after local back pain has improved.
Patients may notice that their severe back pain is largely gone but still experience intermittent tingling, numbness, or discomfort farther down the leg.
The same can occur with a cervical disc injury affecting an arm or hand.
This is one reason pain alone shouldn't be the only measurement of recovery.
Neurological function and overall activity tolerance also matter.
Does a Follow-Up MRI Prove the Disc Is Healing?
MRI can provide useful information, but another MRI isn't automatically necessary simply because you're recovering.
Imaging is only part of the picture.
Dr. Boelk may also consider:
-
How your symptoms have changed
-
Where your symptoms are located
-
Muscle strength
-
Sensation
-
Neurological findings
-
Orthopedic findings
-
Sitting tolerance
-
Standing tolerance
-
Walking tolerance
-
Ability to perform normal activities
The MRI should be interpreted in the context of the patient standing in front of you.
What If My Herniated Disc Isn't Healing?
If you've given the condition appropriate time and treatment but symptoms remain significant, it may be worth reevaluating why.
Possible questions include:
-
Is the disc actually producing the symptoms?
-
Is a spinal nerve still being irritated?
-
Is there spinal or foraminal stenosis?
-
Is spinal instability present?
-
Are certain movements repeatedly aggravating the condition?
-
Is another structure contributing to the symptoms?
-
Does the treatment approach need to change?
For appropriate patients, Non-Surgical Spinal Decompression Therapy may be another conservative option to consider.

Determining whether someone is actually an appropriate candidate is more important.
Not Every Patient Is a Candidate
Having a herniated disc on an MRI doesn't automatically mean you should receive Non-Surgical Spinal Decompression.
Some patients require additional diagnostic testing.
Others may be better suited for another conservative treatment.
And sometimes surgery really is the appropriate recommendation.
Jeff traveled from Tucson hoping to find another option for his condition.
After his evaluation, he and Dr. Boelk determined otherwise.
⭐⭐⭐⭐⭐
“I found this terrific practice on social media. I followed up and did my own research so I decided to pay them a visit. The place is clean, well organized and run by a very efficient receptionist. I met with the doctor and after a honest and forthright conversation, we both determined that surgery was my only option to correct my back issues. I will return as a customer when the time is right.”

That's why patient selection matters.
The goal isn't to recommend spinal decompression to everyone.
It's to determine whether you're an appropriate candidate.

When Is a Herniated Disc NOT Improving Normally?
Good and bad days can occur during recovery.
But there is an important difference between a temporary flare-up and progressively worsening neurological symptoms.
Seek prompt medical evaluation for increasing weakness, worsening numbness, or significant deterioration in your ability to walk or function.
Seek emergency medical attention for symptoms including:
-
New loss of bowel or bladder control
-
Numbness in the groin or saddle region
-
Rapidly progressing muscle weakness
-
Significant or rapidly worsening neurological deficits
These symptoms may indicate serious nerve compression and require immediate evaluation.
