The First Thing to Understand Is That Sciatica Is a Symptom
One of the biggest reasons patients become frustrated is because they're trying to answer the wrong question.
They ask:
"How long does sciatica last?"
A better question is:
"What's causing my sciatica?"
Sciatica isn't actually a diagnosis.
It's a description of pain, numbness, tingling, burning, or weakness caused by irritation of the sciatic nerve.
That irritation may come from:
- A herniated disc
- A bulging disc
- Lumbar spinal stenosis
- Degenerative disc disease
- Foraminal stenosis
- Spondylolisthesis
- Or several other spinal conditions
Each has its own recovery timeline.
That's why one person's sciatica resolves within a few weeks while another person's symptoms continue for months.
The timeline depends much more on the cause than on the word "sciatica."
Why Do Some People Recover Quickly While Others Don't?
This is one of the questions I hear most often.
The answer isn't always straightforward, but several factors play a role:
- The underlying diagnosis.
- How long the nerve has been irritated.
- The size and location of a disc injury (if present).
- Overall health.
- Smoking status.
- Activity level.
- Whether the condition is improving or continuing to worsen.
One important fact many patients don't know is that spinal discs have a very limited direct blood supply.
Unlike muscles, which receive abundant blood flow after an injury, discs rely largely on the movement of nutrients and oxygen through the disc itself.
Because of this, disc injuries often heal much more slowly than people expect.
That's one reason sciatica sometimes improves gradually rather than disappearing overnight.
A Common Misconception
One of the biggest myths surrounding sciatica is:
"If it hasn't gone away after a month, it'll never go away."
Fortunately...
That simply isn't true.
I've evaluated many patients who struggled for months—or even years—before finally receiving an accurate diagnosis and an appropriate treatment plan.
Time alone doesn't determine whether improvement is possible.
The more important question is:
Why hasn't it improved yet?
Sometimes the answer is surprisingly straightforward.
What If Your Sciatica Hasn't Gone Away?
If your sciatica has been present for several weeks—or keeps returning despite previous treatment—it doesn't automatically mean you're destined for surgery.
It does mean it's time to ask a different question.
Instead of asking,
"How do I get rid of my pain?"
Ask,
"Why is my sciatic nerve still irritated?"
Those are two very different questions.
Pain is the symptom.
The underlying problem is the diagnosis.
Until the diagnosis is understood, it's difficult to know which treatment has the greatest chance of helping.
That's why some people bounce from treatment to treatment without ever getting lasting relief.
Does Sciatica Always Require Surgery?
Fortunately, no.
This is one of the biggest fears patients have.
They assume that because the pain has lasted several months—or because an MRI shows a herniated disc—surgery must be the next step.
Fortunately, that isn't always the case.
Many patients improve with appropriate conservative care.
Others eventually require surgery because of progressive weakness, severe neurological compromise, or symptoms that simply fail to improve despite appropriate treatment.
