Before You Schedule Surgery, Ask These Questions

Severe pain can make surgery feel like the fastest way out.

Sometimes it is.

But before moving forward, several questions should be answered:

  • Is muscle weakness present or getting worse?

  • Is the spine stable?

  • Have reasonable non-surgical options been explored?

  • Is surgery urgent, or do you have time to consider another approach?

A herniated disc is an important finding, but it does not automatically mean every patient needs an operation.

Does the Herniated Disc Match Your Symptoms?

This is one of the first questions Dr. Boelk considers.

Many adults have disc bulges or age-related changes that appear on an MRI but do not cause pain. Finding a herniated disc does not automatically prove that it is responsible for the patient’s symptoms.

The location of the pain should make sense when compared with the injured spinal level.

For example, a lumbar disc injury may cause lower-back pain, sciatica, numbness in the foot, or weakness in the leg. A cervical disc injury may cause neck or shoulder-blade pain, symptoms traveling down the arm, hand numbness, or reduced grip strength.

Dr. Boelk compares the MRI with the patient’s pain pattern, neurological findings, muscle strength, reflexes, and movements that aggravate or relieve the symptoms.

When these findings line up, there is a stronger reason to believe the disc is involved.

Is the Nerve Irritated—or Is It Losing Function?

Pain can be severe even when a nerve is still functioning normally. That does not make the pain unimportant, but it may allow time to consider non-surgical treatment.

Progressive muscle weakness is different.

A patient who is increasingly unable to lift the foot, grip an object, raise an arm, or control a leg may need more immediate neurological or surgical evaluation.

Dr. Boelk also watches for:

  • Increasing numbness

  • Changes in reflexes

  • Loss of coordination

  • Difficulty walking

  • Symptoms spreading farther into the arm or leg

  • Loss of bladder or bowel control

  • Numbness in the groin or saddle region

The last two symptoms require urgent medical attention and should not be managed as a routine disc problem.

Is the Spine Stable?

Spinal stability is an important part of deciding whether decompression can be performed safely.

An MRI is usually taken while the patient is lying down. It may show the discs and nerves clearly, but it does not always reveal how the spine behaves while standing or moving.

When indicated, Dr. Boelk may use weight-bearing and flexion-extension X-rays to look for abnormal movement.

If the spine is unstable, decompression may not be appropriate without additional evaluation or clearance.

What If You Have Already Tried Physical Therapy or Injections?

Many patients assume that surgery is the only remaining choice once medication, physical therapy, injections, or chiropractic adjustments have failed.

That is not necessarily true.

Those treatments may have been reasonable, but they do not all address the spine in the same way. A patient may still have a disc-related problem that has not responded because pressure around the injured area has not been adequately addressed.

This does not mean spinal decompression will work for everyone who has failed another treatment. It means that a lack of improvement with one approach does not automatically answer whether another reasonable non-surgical option is available.

How Non-Surgical Spinal Decompression Is Different

Non-surgical spinal decompression uses controlled pulling and releasing cycles to reduce pressure at a targeted spinal level.

The machine is only one part of the process. Patient selection, treatment frequency, proper settings, daily activity, and rehabilitation all matter.

Surgery Can Be the Right Decision

The goal is not to persuade every patient to avoid surgery.

Surgery may be appropriate when:

  • Weakness is rapidly progressing

  • A neurological emergency is present

  • The spine is unstable

  • The condition cannot be treated safely with decompression

  • A reasonable course of non-surgical treatment has failed

  • A surgeon determines that delaying the procedure could risk permanent nerve damage

A responsible non-surgical doctor should be willing to say when a patient needs a different type of treatment.

Previous Surgery and Hardware Can Change the Decision
