What Is a Herniated Disc?

Between each vertebra is a spinal disc that acts as a shock absorber.

  • A tough outer layer (annulus fibrosus)

  • A softer, gel-like center (nucleus pulposus)

When the outer fibers weaken or tear, part of the inner material can push outward. This is called a herniated disc.

If that disc material irritates or compresses a nearby nerve, symptoms can include:

Interestingly, not every herniated disc causes pain. Many people have disc herniations on MRI and never develop symptoms. That's why it's critical that your MRI findings match your physical examination before deciding on treatment.

Does Every Herniated Disc Need Surgery?

No.

Most people are surprised to learn that surgery is not the first recommendation for the majority of herniated discs.

Depending on your examination, MRI findings, overall health, and severity of symptoms, conservative treatment may include:

  • Activity modification

  • Physical rehabilitation

  • Anti-inflammatory strategies

  • Guided exercise

  • Lifestyle changes

However, healing often takes time, and many patients continue experiencing pain because the irritated nerve remains under mechanical stress during everyday activities such as sitting, bending, lifting, or prolonged driving.

For carefully selected patients, reducing pressure on the injured disc and affected nerve may help create a more favorable environment for recovery.
