Yes. You can absolutely have sciatica without having much lower-back pain, and in some cases the back may not hurt at all.
This throws a lot of people off because they assume that if the problem were coming from their lower back, their back would have to hurt. That's simply not always what happens with an irritated or compressed nerve.

One person may have pain in the buttock and down the back of the thigh. Another may have burning in the calf. Someone else may have tingling or numbness in the foot with surprisingly little actual pain.
That doesn't mean every pain in the buttock, thigh or calf is coming from the spine. It means the absence of back pain isn't enough to rule it out.

How Can the Problem Be in Your Back When Your Back Feels Fine?
This actually makes more sense when you think about how nerves work.
If you irritate a nerve in one location, you can feel symptoms somewhere else along that nerve.
The lumbar nerve roots that contribute to the sciatic nerve begin in the lower spine. If one of those nerve roots is irritated or compressed, you may feel pain, burning, tingling or numbness farther down the pathway.
That's why Dr. Boelk may be very interested in your lower back even though you keep pointing to your leg.
It's also why repeatedly treating the spot that hurts doesn't always fix the problem.
A patient may spend months stretching a hamstring because the back of the leg constantly feels tight. Someone else keeps working on the piriformis because most of the pain is in the buttock. Another patient assumes it's a calf problem because that's where the burning is the worst.
Sometimes they're right.
Sometimes they're treating the place where the symptoms ended up rather than the place where they started.
That's what needs to be sorted out.
Show Dr. Boelk Exactly Where the Symptoms Go
When someone tells Dr. Boelk, “My leg hurts,” he'll usually ask them to be much more specific.
For example:

Does it begin in the buttock?

Does it run down the back or outside of the thigh?

Does it stop at the knee?

Does it continue into the calf?

Is the top of the foot numb?

Is it the outside of the foot?

Are certain toes tingling?

Does the leg feel weaker than the other side?
Different lumbar nerves are associated with different areas of sensation and muscle function, so the pattern can provide useful information.
But Dr. Boelk doesn't diagnose someone by looking at a dermatome chart and deciding that one numb toe automatically means a particular disc is the problem.
After treating these conditions for more than two decades, he knows that real patients don't always follow textbook patterns perfectly.
The symptom pattern is one piece of the puzzle. The history, examination, strength, sensation, movements that aggravate the problem and available imaging are other pieces.
They should make sense together.
The MRI Needs to Match the Person Standing in Front of It
This is another area where patients can get confused.
A lumbar MRI may show a disc bulge at one level, degeneration somewhere else, a herniation at another level and narrowing around one or more nerves.
It's easy to read that report and think every abnormality must be responsible for your symptoms.
That's not necessarily the case.
Dr. Boelk wants to know whether what appears on the MRI actually helps explain what you're feeling.
If all of your symptoms are running down the right leg, for example, does the MRI show something that could reasonably affect the nerve on that side?
If part of your foot is numb or a particular movement has become weak, does that make sense with what appears on the images?
That's why Dr. Boelk prefers patients to bring the actual MRI images along with the written report whenever they're available.
The report matters. The images matter. Most importantly, the patient matters.
Dr. Boelk doesn't treat an MRI report by itself.

Pain gets people's attention. Numbness and weakness sometimes don't.
A patient may tell Dr. Boelk that the pain has improved and then casually mention that half of the foot is still numb.
That's important information.
Pay attention if numbness is persistent or spreading, one leg feels weaker than the other, you're having difficulty lifting the front of the foot, the foot starts slapping the ground, or you're beginning to catch your toes when you walk.
Less pain doesn't necessarily mean everything is improving if neurological symptoms are progressing.
Most cases of sciatica are not emergencies. Rapidly progressing weakness, new bowel or bladder dysfunction, difficulty urinating, or numbness around the groin or saddle region requires prompt medical evaluation rather than waiting for a routine spinal decompression consultation.

There are also situations where decompression may not be appropriate, including an unhealed fracture, significant osteoporosis, unstable spondylolisthesis, abdominal aortic aneurysm, pregnancy for lumbar decompression, certain malignancies and other medical or structural conditions.
Previous spinal surgery needs to be evaluated carefully as well, particularly when fusion or metal hardware is present at the level that would need treatment.
