Can Spinal Decompression Help an L5-S1 Disc Herniation? | Mira Mesa, CA

An L5-S1 disc herniation can be frustrating, especially when the pain starts traveling down your leg. Some people have trouble sitting at work, others find it difficult to stand or walk for very long, and some wake up every morning wondering how bad their back or sciatica will be that day.

One of the questions patients frequently ask is whether a herniated disc at L5-S1 can improve without surgery.

In many cases, it can. But the right treatment depends on what's happening with the disc, whether a nerve is involved, and how the spine is functioning overall.

Non-Surgical Spinal Decompression may be an option for certain patients with L5-S1 disc herniations. The important part is determining whether someone is an appropriate candidate before beginning treatment.

Dr. Cassidy James Boelk, DC, has spent more than 22 years specializing in spinal decompression and helping patients with complicated disc conditions. His practice, The Spinal Decompression & Chiropractic Center of San Diego, is located in Kearny Mesa, a short drive south of Mira Mesa.

Over the years, he's worked with patients suffering from disc herniations, sciatica, spinal stenosis, degenerative disc disease, and other conditions that can make everyday activities difficult.

Wondering Whether Spinal Decompression Could Help?

Dr. Boelk offers a limited number of No-Charge Consultations each week for patients interested in learning whether spinal decompression may be appropriate for their condition.

During the consultation, patients can discuss their symptoms, learn about the DRX9000 spinal decompression system, and find out what information is needed to determine candidacy.

If a more comprehensive examination or additional imaging is needed, the office can explain the next steps.

When a Herniated Disc Starts Interfering With Your Life

One of the difficult things about disc-related pain is how much it can affect ordinary activities.

Something as simple as getting dressed, driving to work, or sitting through dinner can become uncomfortable. Patients sometimes find themselves avoiding exercise, canceling plans, or taking medications just to get through the day.

Anthony K., a San Diego patient who previously underwent surgery for a bulging disc, described his experience when his symptoms returned.

⭐⭐⭐⭐⭐

“I have been dealing with back pain for several years. It got so bad that I had surgery 5 years ago for a bulging disc. About 5 months ago the disc bulged again hitting my sciatic nerve creating pain in my legs that made me want to cry it hurt so bad. After trying so many other options I was fortunate enough to find the spinal decompression and chiropractic center of San Diego. After just a few sessions I noticed a difference. Now after being there for the past few months I'm able to go about my normal life again.

What a blessing it was to find a Dr that genuinely cares and knows how to help people in my situation. Thank you Dr Boelk!”

What Makes the L5-S1 Disc Different?

L5-S1 is the lowest disc in the lumbar spine. It sits between the fifth lumbar vertebra and the sacrum, where the lower back connects to the pelvis.

Because of its location, this disc is exposed to significant mechanical forces throughout the day.

When you bend forward, lift something, twist, or sit for extended periods, the lower lumbar discs help absorb and distribute those forces.

A disc herniation occurs when material from the softer inner portion of the disc pushes through a weakened area of its outer layer.

Sometimes that herniation causes lower back pain. Other times, it irritates or compresses a nearby spinal nerve, leading to symptoms that travel into the leg.

It's also possible to have an L5-S1 disc herniation without significant pain.

That's why Dr. Boelk looks beyond the MRI report. He wants to know whether the findings actually match the patient's symptoms and examination.

Why Does an L5-S1 Disc Herniation Cause Sciatica?

At the L5-S1 level, a herniated disc commonly affects the S1 nerve root.

When this nerve becomes irritated, pain may travel from the buttock down the back of the thigh and calf, sometimes reaching the outside of the foot.

Some patients also experience numbness, tingling, or weakness when trying to rise onto their toes.

Depending on the location of the herniation, the exiting L5 nerve root may be affected instead. This can produce a different pattern of symptoms, sometimes involving the outer leg or top of the foot.

The location of pain can provide useful clues, but it doesn't tell the whole story.

Dr. Boelk also evaluates muscle strength, sensation, reflexes, spinal movement, and MRI findings to better understand which nerve may be involved.

How Does the DRX9000 Spinal Decompression System Work?

Non-Surgical Spinal Decompression uses controlled mechanical traction to apply carefully regulated forces to the spine.

The goal is to reduce mechanical loading on selected spinal structures and potentially decrease irritation associated with certain disc conditions.

At The Spinal Decompression & Chiropractic Center of San Diego, Dr. Boelk exclusively uses DRX9000 spinal decompression systems.

Unlike hanging upside down on an inversion table or performing general stretching exercises, the DRX9000 allows treatment parameters to be adjusted and controlled throughout the session.

Some patients report improvements in lower back pain, sciatica, and daily function during treatment.

However, spinal decompression is not a guaranteed solution. Research into motorized spinal decompression remains mixed, and it has not been established that treatment consistently reverses disc herniations or prevents surgery.

For Dr. Boelk, proper patient selection and monitoring progress are essential parts of the process.

What If You've Already Tried Other Treatments?

Many patients who contact Dr. Boelk have already spent months trying to find relief.

They may have completed physical therapy, tried medications, received epidural steroid injections, or undergone chiropractic treatment.

Sometimes these approaches help temporarily, but the symptoms return.

Other patients improve their strength and flexibility yet continue experiencing significant nerve pain.

When that happens, Dr. Boelk believes it's important to reconsider the diagnosis rather than simply repeat treatments that haven't helped.

Michael Langsdale, a San Diego patient who experienced persistent sciatica, shared his experience.

⭐⭐⭐⭐⭐

“I have had an outstanding experience with Dr. Boelk and his staff at The Spinal Decompression & Chiropractic Center of San Diego.

They follow a very structured treatment protocol and utilize the best spinal decompression equipment available. I've been treated elsewhere using lower-quality decompression systems and never experienced meaningful improvement.

Dr. Boelk is attentive, professional, and genuinely cares about every patient. You can see it in the way he interacts with everyone who walks through the door.

After about 12 treatments, my sciatica was gone and I was nearly back to normal. Honestly, I wasn't sure I would ever heal. What surprised me most was that after my very first treatment, I experienced significant relief and was able to sleep mostly pain-free for the first time in months.

The improvement has allowed me to get back to hiking, traveling, and enjoying life again.

This treatment was one of the best investments I've ever made.

Thank you, Dr. Boelk and your entire team.”

How Does Dr. Boelk Determine Whether Someone Is a Candidate?

Having an L5-S1 herniated disc doesn't automatically mean spinal decompression is appropriate.

Before recommending treatment, Dr. Boelk evaluates several important factors.

One is whether the patient's symptoms match the MRI findings. A herniated disc may appear significant on imaging but have little to do with the patient's actual complaints.

Another consideration is spinal stability. Some patients have additional conditions, such as spondylolisthesis or advanced degenerative changes, that require further evaluation.

When instability is suspected, standing lumbar X-rays, including flexion and extension views, may provide valuable information.

Neurological findings are also important. Changes in sensation, reflexes, or muscle strength can help determine whether a nerve is being affected and how seriously.

Dr. Boelk also reviews previous treatment history, current symptoms, and the patient's overall health before making recommendations.

Why Some Patients Are Not Accepted for Spinal Decompression

Dr. Boelk does not recommend spinal decompression for every patient.

Certain conditions may make treatment inappropriate, including unstable spondylolisthesis, certain fractures, spinal fusion hardware at the intended treatment level, and other serious spinal or medical conditions.

Significant or progressive neurological weakness may require urgent medical or surgical assessment.

New bladder or bowel dysfunction, saddle anesthesia, or rapidly progressive leg weakness can indicate cauda equina syndrome or another serious neurological condition and require immediate emergency evaluation.

Sometimes a patient is better served by seeing a spine surgeon or another specialist.

Jan H., a patient from San Marcos, shared his experience after Dr. Boelk determined that treatment wasn't appropriate while a cervical fracture was healing.

⭐⭐⭐⭐⭐

“I decided to see Dr. Boelk after a pretty gnarly motocoss neck injury. At the Hospital I was at, I felt I was getting pressured into a surgery which I refused. A friend of a friend referred him after Dr. Boelk successfully treated his broken back.

Dr. Boelk is a compassionate individual who truly cares about his patients. He looked at my MRI images, explained what he saw and potential routes for recovery. He could not treat me as I have to wait until the fracture in my neck heals, but the level of professionalism, compassion, caring and knowledge displayed by him was exceptional.

He also recommended to seek an additional opinion form another neurosurgeon. My experience was that he really cares and he's not someone who would take advantage of the situation someone's in (unlike some other doctors). I will definitely be back and already recommended him to others as well! Thank you Dr. Boelk!”

— Jan H., San Marcos, CA

Although Jan's condition involved his neck rather than L5-S1, his experience illustrates why determining candidacy is an important part of responsible patient care.

Can an L5-S1 Disc Herniation Improve Without Surgery?

Yes. Many lumbar disc herniations improve with conservative care, and some decrease in size naturally over time.

For patients without urgent neurological findings, treatment may include activity modification, carefully selected exercises, physical therapy, medications, and other non-surgical approaches.

Spinal decompression may also be considered in selected cases, although current research has not established that it reliably prevents the need for surgery.

The important thing is to determine whether conservative care can be pursued safely.

If significant neurological deficits or other serious findings are present, delaying appropriate medical or surgical evaluation may not be advisable.

⭐⭐⭐⭐⭐

“Hi my name is Rosa Algravez. I have suffered from leg numbness and low back pain for 4 years. I haven’t been able to do groceries or even take baths without pain. I looked for relief with various doctors with no luck. I have done treatments ranging from chiropractic to acupuncture. My husband found Dr. Boelk in the newspaper and we decided since we had tried everything else that we were going to give this a try. After my first few treatments, I feel as though I have returned to 100%. Life for me is completely different.

My husband and I have returned to enjoying life. We travel 300+ miles everyday from Calexico. I can do things now with little to no limitations. The staff makes you feel welcomed and it’s a very family environment. The payments are easy and I would recommend this to anyone suffering with low back pain. Thank You”

An appropriate clinical evaluation is still necessary before determining whether someone qualifies for treatment.

Common Questions About L5-S1 Disc Herniations

Can an L5-S1 herniated disc cause numbness in the foot?

Yes. Depending on which nerve root is involved, an L5-S1 herniation may cause numbness, tingling, or weakness in parts of the foot.

Why does my L5-S1 herniated disc hurt more when I sit?

Sitting can increase symptoms for some patients with lumbar disc herniations, particularly when combined with forward bending. However, not everyone responds the same way.

Can I continue exercising with an L5-S1 disc herniation?

Many patients can remain active with appropriate modifications. The best exercises depend on symptoms, neurological findings, spinal stability, and individual tolerance.

Can spinal decompression help if physical therapy hasn't worked?

It may be considered in selected cases, but the reason physical therapy was unsuccessful should be evaluated first. Decompression is not guaranteed to work simply because another treatment didn't help.

How long does it take to recover from an L5-S1 disc herniation?

Recovery varies considerably. Some patients improve within weeks, while others experience symptoms for months. The severity of nerve involvement and other clinical findings can influence recovery.
