Can Spinal Decompression Help an L5-S1 Disc Herniation? | La Jolla, CA

If you've been diagnosed with an L5-S1 disc herniation, there's a good chance you've already tried changing how you sit, sleep, exercise, or move throughout the day.

Maybe sitting through dinner has become uncomfortable. Perhaps you're avoiding golf, surfing, or your usual workouts because certain movements trigger pain down your leg. Or maybe the pain has become so persistent that you're starting to wonder whether surgery is your only option.

Non-Surgical Spinal Decompression may help certain patients with an L5-S1 disc herniation, but the first step is determining whether the disc is actually responsible for your symptoms.

Dr. Cassidy James Boelk, DC, has spent more than 22 years specializing in spinal decompression and helping patients with complicated disc conditions explore conservative treatment options.

As a Certified Spinal Decompression Doctor and Certified Pain-Free Performance Specialist, he has extensive experience evaluating patients with herniated discs, sciatica, degenerative disc disease, and other spinal conditions.

Patients from La Jolla, University City, Pacific Beach, and throughout San Diego visit his Kearny Mesa practice for specialized spinal decompression evaluations.

A Patient's Experience With Severe Sciatica

One of the frustrating things about sciatica is how much it can interfere with everyday life.

For some patients, even sleeping or sitting comfortably becomes difficult.

Toni H., a San Diego patient who had struggled with sciatica and back pain for years, shared her experience:

⭐⭐⭐⭐⭐

“For the first time in 10 years, I am totally pain free. My sciatica is gone and I have no more back pain! I've tried other chiropractic procedures, acupuncture, pain killing drugs, and other type therapies that just did not work. I was one step before back surgery, until I discovered Dr. Boelk's therapy and his procedure, and it's a miracle! My office visits were always so relaxing and comfortable, there was never any pain involved with the therapy. Dr. Boelk and Patty are so welcoming and warm and ready to explain everything that's going on. They both take such good care of you! Thank you Dr. Boelk and Patty!”

Why Is an L5-S1 Herniated Disc So Painful?

L5-S1 is the lowest disc in the lumbar spine, sitting between the fifth lumbar vertebra and the sacrum.

It's an area that handles a considerable amount of stress during everyday activities.

When you bend over, lift something, twist, or sit for long periods, forces are transferred through this region.

Over time, or sometimes following an injury, the outer portion of the disc can weaken. When material from inside the disc pushes through that weakened area, it's called a disc herniation.

The herniation itself isn't always painful.

The bigger concern is what happens when disc material irritates or compresses a nearby nerve.

Patients may experience lower back pain, but they can also develop symptoms that travel much farther.

Common complaints include:

  • Pain shooting from the buttock into the back of the leg.

  • Numbness or tingling in the calf or foot.

  • Burning or electric-like sensations.

  • Difficulty sitting for extended periods.

  • Increased pain when bending forward.

  • Weakness in certain leg or foot muscles.

Interestingly, some patients have an L5-S1 herniation on MRI without experiencing significant symptoms.

Others have relatively small herniations that produce considerable nerve irritation.

This is why Dr. Boelk doesn't recommend treatment based on MRI findings alone.

L5-S1 Disc Herniation and Sciatica: Which Nerve Is Involved?

An L5-S1 disc herniation frequently affects the S1 nerve root.

When the S1 nerve becomes irritated, pain may travel from the buttock down the back of the thigh and calf, sometimes reaching the outside of the foot.

Some patients also notice weakness when attempting to rise onto their toes.

However, not every L5-S1 herniation affects the same nerve.

Depending on where the disc material is located, the exiting L5 nerve root may be involved instead.

That can create a different pattern of pain, numbness, or weakness.

Dr. Boelk evaluates the patient's symptoms, reflexes, sensation, muscle strength, and MRI findings to better understand which structures may be responsible.

The location of the herniation matters, but the neurological examination is just as important.

How Can the DRX9000 Help an L5-S1 Herniated Disc?

Non-Surgical Spinal Decompression uses controlled mechanical traction to apply carefully regulated forces to the spine.

The goal is to reduce mechanical loading on certain spinal structures and potentially decrease irritation associated with disc-related conditions.

At The Spinal Decompression & Chiropractic Center of San Diego, Dr. Boelk exclusively uses DRX9000 spinal decompression systems.

The clinic has seven decompression tables, allowing the team to accommodate patients with both cervical and lumbar conditions.

The DRX9000 allows treatment parameters to be adjusted according to the patient's condition and response.

This is different from simply hanging upside down on an inversion table or performing general stretching exercises.

Some patients experience improvements in back pain, leg symptoms, and daily activities during treatment.

However, spinal decompression cannot guarantee that a herniated disc will return to its original position or that nerve compression will resolve. Clinical evidence remains mixed, and additional high-quality research is needed to establish which patients are most likely to benefit.

For Dr. Boelk, the important question is whether decompression is appropriate for the individual patient.

What If You've Already Tried Physical Therapy or Injections?

Many patients who visit Dr. Boelk have already tried several treatments.

They may have completed weeks of physical therapy, taken medications, tried chiropractic adjustments, or received epidural steroid injections.

Sometimes these treatments help temporarily.

Other times, patients become stronger and more flexible but continue experiencing the same pain down their leg.

When this happens, Dr. Boelk believes the diagnosis deserves another careful look.

Is the L5-S1 disc actually causing the symptoms? Is another spinal level involved? Could spinal instability or foraminal stenosis be contributing?

These are important questions before deciding on another treatment.

Steve Robinson, a patient who underwent spinal decompression treatment, described his experience:

⭐⭐⭐⭐⭐

“Dr. Boelk is professional, kind, and truly prioritizes his patients’ health. The spinal decompression treatments, combined with the exercises and guidance he provided, have reduced my back pain to the point that I can sleep again.

Traditional chiropractic adjustments did not help my lower back—but spinal decompression did.

I highly recommend Dr. Boelk.”

— Steve Robinson

Who Is a Good Candidate for L5-S1 Spinal Decompression?

Not everyone with an L5-S1 herniation should undergo spinal decompression.

Dr. Boelk considers several factors before recommending treatment.

First, the symptoms should make sense with the MRI. A herniated disc that doesn't correlate with the patient's symptoms may not be the source of the problem.

Second, spinal stability matters. Some patients have additional conditions such as spondylolisthesis, degenerative disc disease, or significant facet joint changes. Standing lumbar X-rays, including flexion and extension views, may be appropriate when instability is suspected.

Third, neurological findings must be considered. Numbness, tingling, reflex changes, and muscle weakness help determine the severity of nerve involvement.

Finally, Dr. Boelk considers the patient's medical history, previous treatments, and overall condition.

A patient who has been experiencing intermittent sciatica for several months may require a very different approach than someone who has recently developed significant muscle weakness.

When Spinal Decompression Is NOT the Right Treatment

Dr. Boelk does not accept every patient for spinal decompression.

Certain conditions may make treatment inappropriate, including significant spinal instability, unstable spondylolisthesis, certain fractures, spinal fusion hardware at the intended treatment level, and other medical contraindications.

Patients with significant or progressive neurological weakness may require urgent specialist evaluation.

New bladder or bowel dysfunction, saddle anesthesia, or rapidly progressive leg weakness can indicate a serious neurological emergency and require immediate emergency medical evaluation.

Sometimes the appropriate recommendation is a surgical consultation rather than spinal decompression.

One patient, Toni W. from Lake Elsinore, appreciated Dr. Boelk's honesty after learning that her extensive spinal hardware made her an unsuitable candidate.

⭐⭐⭐⭐⭐

“Very nice Dr. and Honest ! I would definitely recommend anyone who hasn't had metal put in there back in a surgery to go to him and give his program a shot. I believe it would definitely help. Just not in my personal case. As I have a lot of hardware in my back. But I'm glad I took the time to meet with him. It was very informative all around.”

— Toni W., Lake Elsinore, CA

For Dr. Boelk, determining when not to treat someone is an important part of responsible patient care.

Can an L5-S1 Herniated Disc Get Better Without Surgery?

Yes. Many lumbar disc herniations improve with conservative treatment, and some decrease in size naturally over time.

Treatment may include activity modification, carefully selected exercises, physical therapy, medications, and other non-surgical approaches.

Spinal decompression may be considered for selected patients, although current research has not established that it reliably prevents surgery.

Dr. Boelk's goal is to help patients explore appropriate conservative options when it is safe to do so.

If surgery is the better option, patients should receive that recommendation.

Frequently Asked Questions About L5-S1 Disc Herniations

Can an L5-S1 herniated disc cause pain all the way into my foot?

Yes. Depending on which nerve root is affected, an L5-S1 disc herniation can cause pain, numbness, or tingling extending into the foot.

Is walking good for an L5-S1 herniated disc?

Walking is often a reasonable activity when tolerated. However, patients should adjust the amount of walking based on their symptoms. Increasing neurological symptoms should prompt reassessment.

Can spinal decompression help an L5-S1 disc extrusion?

It may be considered in selected cases, but the type of disc herniation, neurological findings, spinal stability, and overall clinical presentation must be evaluated first.

How many spinal decompression treatments will I need?

There is no universal number. Dr. Boelk develops individualized treatment plans and periodically reassesses progress to determine whether continuing treatment is appropriate.

Can I exercise with an L5-S1 herniated disc?

Many patients can continue exercising with appropriate modifications. The best exercises depend on symptoms, neurological findings, spinal stability, and which movements aggravate or relieve the condition.
