Can Spinal Decompression Help an L5-S1 Disc Herniation? | Chula Vista, CA
One of the most frustrating things about an L5-S1 disc herniation is that the pain doesn't always stay in your lower back. You might feel reasonably comfortable standing, but the moment you sit down, pain starts traveling through your buttock and down your leg. Or perhaps sitting feels fine, but walking, bending, or getting out of bed brings on the symptoms.
Some patients describe a deep ache. Others experience burning, tingling, numbness, or sharp, electric-like pain that travels all the way into the foot.
If you've been diagnosed with an L5-S1 disc herniation, you may have already tried physical therapy, medications, chiropractic treatment, or epidural injections. And if those haven't provided lasting relief, it's understandable to start wondering what comes next.
For certain patients, Non-Surgical Spinal Decompression may be worth considering before moving on to more invasive treatment. But the important question isn't simply whether you have a herniated disc. It's whether that disc is causing your symptoms and whether spinal decompression is appropriate for your particular condition.
Dr. Cassidy James Boelk, DC, has spent more than 22 years working with patients who have complicated cervical and lumbar disc problems. At The Spinal Decompression & Chiropractic Center of San Diego, he evaluates patients from Chula Vista and throughout South County who want to better understand their options.

When a Herniated Disc Starts Affecting Everyday Life
A herniated disc can change the way someone goes through their entire day.
Patients sometimes describe planning their activities around how long they can sit, whether they'll be able to walk comfortably, or how much pain they'll experience afterward. Even getting dressed in the morning can become difficult.
Paul C., a patient who came to Dr. Boelk with a disc herniation, described what he was experiencing and how he responded to treatment.
⭐⭐⭐⭐⭐
“I have been seeing Dr. Boelk since November for my back pain due to disc herniation. I lost mobility and had difficulty even putting shoes on. The pain was interfering with my everyday lifestyle. After reviewing the X-ray results, we agreed upon a treatment plan. Agreeing to this treatment plan was the best choice I've made for my back pain. The spinal decompression has relieved all the pain I felt in my lower back. I can walk, run and exercise without any pain and I'm able to get back in the gym and continue my daily routines. The office staff are very friendly and caring as well. I highly recommend Dr. Boelk's office to anyone seeking exceptional care.”

What Actually Happens When the L5-S1 Disc Herniates?
The L5-S1 disc sits at the very bottom of the lumbar spine, between the fifth lumbar vertebra and the sacrum.
This is an area that handles a considerable amount of mechanical stress. Every time you bend, lift, twist, or transition from sitting to standing, forces pass through the lower lumbar spine.
The disc itself has a tougher outer layer and a softer center. When the outer portion becomes damaged or weakened, material from inside the disc can push outward. That's what we call a disc herniation.
But here's something many patients don't realize: having a disc herniation doesn't automatically mean it's the source of your pain.
It's not unusual for an MRI to show a herniated disc in someone who has very few symptoms.
The problem becomes more concerning when the herniation irritates or compresses a nearby nerve.
At L5-S1, the S1 nerve root is commonly involved. Patients may experience pain traveling down the back of the thigh and calf, sometimes reaching the outside of the foot. Some also notice numbness, tingling, changes in reflexes, or weakness when trying to rise onto their toes.
Depending on the location of the herniation, the L5 nerve root may be affected instead.
This distinction matters because different nerves produce different patterns of symptoms. Understanding those patterns, along with the examination and MRI findings, helps determine what treatment makes sense.
So Where Does Spinal Decompression Fit In?
Non-Surgical Spinal Decompression uses a specialized motorized table to apply controlled traction forces to the spine.
The purpose is to reduce mechanical loading on certain spinal structures and potentially decrease irritation associated with disc problems.
Dr. Boelk exclusively uses the DRX9000 spinal decompression system. His San Diego practice has seven decompression tables, and treatment parameters can be adjusted according to the patient's condition and response.
It's different from simply stretching your back or hanging upside down on an inversion table. The equipment allows the clinician to control the amount and timing of traction.
Some patients report meaningful improvements in their back pain, leg symptoms, and ability to function during treatment.
That said, spinal decompression isn't a guaranteed solution. It hasn't been established that motorized decompression consistently pulls herniated discs back into place, and research on its effectiveness remains mixed.
Dr. Boelk's approach is to evaluate the patient carefully, determine whether decompression is a reasonable option, and monitor the response rather than assume every herniated disc will improve.
What If You've Been Dealing With Sciatica for Months?
One of the biggest challenges with persistent sciatica is that patients often begin limiting their activities because they're afraid of making the problem worse.
They stop exercising. They avoid sitting for long periods. Some begin having difficulty sleeping or performing their normal job duties.
Joshua T., a San Diego firefighter, described what happened after dealing with persistent lower back pain and sciatica.
⭐⭐⭐⭐⭐
“Dr. Boelk has been a lifesaver.
I suffered from constant low back pain and sciatica for more than eight months. Pain medications didn't help, and I was headed toward multiple injections in hopes of finding relief.
After beginning treatment with Dr. Boelk, everything changed.
Today, my pain is completely gone.
As a firefighter who depends on his body every day to help others, being pain-free has made a tremendous difference in both my career and quality of life.
The results I experienced were absolutely worth it, and I highly recommend looking into what Dr. Boelk and his team can do for you.”
— Joshua T., San Diego, CA
For patients with persistent symptoms, the goal is not simply to reduce pain temporarily. It's also to improve daily function and determine whether the underlying condition can be managed safely without surgery.
Why Dr. Boelk Doesn't Accept Every Patient for Spinal Decompression
This is something Dr. Boelk takes seriously.
A patient may arrive with an MRI showing an L5-S1 herniation, but that doesn't automatically make them a candidate for treatment.
He wants to know whether the symptoms match the imaging, whether the spine is stable, and whether there are neurological findings that suggest a more serious problem.
For example, a patient with an L5-S1 herniation and stable spinal mechanics may be evaluated differently from someone who has significant spondylolisthesis or instability.
In some cases, standing lumbar X-rays, including flexion and extension views, provide information that isn't available from a standard MRI.
Other factors can make spinal decompression inappropriate, including certain fractures, spinal fusion hardware at the intended treatment level, serious medical conditions, and significant or progressive neurological deficits.
Patients with new bladder or bowel dysfunction, saddle anesthesia, or rapidly progressive leg weakness require immediate emergency medical evaluation.
Sometimes surgery is the appropriate recommendation.
A patient from Tucson, Arizona, shared his experience after meeting with Dr. Boelk.
⭐⭐⭐⭐⭐
“I found this terrific practice on social media. I followed up and did my own research so I decided to pay them a visit. The place is clean, well organized and run by a very efficient receptionist. I met with the doctor and after a honest and forthright conversation, we both determined that surgery was my only option to correct my back issues. I will return as a customer when the time is right.”
— Jeff J., Tucson, AZ
For Dr. Boelk, an honest evaluation is more important than recommending treatment to someone who isn't an appropriate candidate.
Does an L5-S1 Herniated Disc Always Require Surgery?
No. In fact, many lumbar disc herniations improve with conservative care, and some decrease in size naturally over time.
Patients may benefit from activity modification, carefully selected exercises, physical therapy, medications, or other non-surgical approaches.
For selected patients, spinal decompression may also be considered, although current research has not established that it reliably prevents surgery.
The key is understanding when conservative care is reasonable and when waiting could create additional risks.
Significant or progressive neurological problems require prompt medical assessment. For other patients, a carefully monitored conservative treatment program may be appropriate.

Traveling to San Diego for Spinal Decompression
Although many patients come from Chula Vista and other parts of San Diego County, some travel much farther to seek Dr. Boelk's expertise.
The office has worked with patients from other states and countries who arrange visits to San Diego for evaluation and, when appropriate, treatment.
Ira Smith, who traveled from Las Vegas, described his experience:
⭐⭐⭐⭐⭐
“My name is Ira Smith. I live in Las Vegas and found out about Dr. Boelk’s program here in San Diego and decided to start treatments with him. I’ve had lower back problems since I was very young. I had aggravated my lower back pretty bad again and usually I could do exercises, chiropractic treatment, rest and this time those things would no longer relieve my pain. I would get the sciatic pain shooting down into my leg and foot and would be pretty bad.
I found out thru my MRI’s that Dr. Boelk went over with me that I had 5 protruding discs. I did not want to get surgery as I have heard and seen that they don’t always work very well at all. I am 2/3 of the way done with Dr. Boelk’s program and I have zero pain. I pretty much go thru my activities with no pain. As I follow the program it has been absolutely fantastic! I have been thru physical therapy, chiropractic programs, I went to the hospital and they told me I have 1 bad disc, but I found out I had 4 more bad discs as well. The treatment has relieved the pain and is not painful at all. This can be the starting point for anyone with back pain to get their life back.
This is the place to start. If your skeptical, what’s your health worth? The cost of this has been way less than the cost of all my other therapies that I have gone thru. This is less money than all those things, less pain and gives you a great quality of life. This is new and modern and it fixes the problem. Use Dr. Boelk’s program to change your life and it will change your life! This is the place to start. -Thank You”
— Ira Smith, Las Vegas, Nevada
Patients traveling from outside San Diego should contact the office before making travel arrangements. The team can explain the evaluation process, scheduling considerations, and what records may be helpful.
Not every out-of-town patient will qualify for treatment.
Common Questions About L5-S1 Herniated Discs
Can an L5-S1 herniated disc cause pain down only one leg?
Yes. Many patients experience symptoms primarily on one side, depending on which nerve root is irritated. However, symptoms can occasionally affect both legs.
Is sitting bad for an L5-S1 herniated disc?
Some patients notice increased symptoms with prolonged sitting, particularly when bending forward. Others tolerate sitting reasonably well. The response depends on the individual condition.
Can an L5-S1 disc herniation heal on its own?
Yes. Some herniated discs decrease in size over time, and many patients improve without surgery. Recovery depends on the severity of symptoms, neurological findings, and other factors.
Can spinal decompression help after physical therapy hasn't worked?
It may be an option for selected patients, but the reason physical therapy didn't help should be evaluated first. Spinal decompression is not guaranteed to be effective simply because another treatment was unsuccessful.
How many spinal decompression treatments are needed?
There is no single number appropriate for everyone. Dr. Boelk develops individualized treatment plans and periodically reassesses whether meaningful progress is occurring.
