"What's the difference, and which one is worse?"
It's one of the most common questions we hear from patients visiting our office from Mira Mesa.
Although the two conditions are related, they are not the same injury.
Both affect the spinal discs.
Both can cause neck pain, back pain, numbness, tingling, or sciatica.
But they develop differently, behave differently, and aren't treated exactly the same.

Understanding Your Spinal Discs
Between each vertebra sits an intervertebral disc.
These discs act like cushions, helping absorb shock while allowing your spine to bend, twist, and move.
Each disc contains two primary structures:
- A tough outer ring called the annulus fibrosus
- A soft, gel-like center called the nucleus pulposus
Over time, discs naturally lose some of their water content and elasticity.
This makes them more susceptible to injury, especially with repetitive bending, lifting, prolonged sitting, or years of normal wear and tear.
What Is a Bulging Disc?
A bulging disc occurs when the disc extends beyond its normal boundaries but does not rupture.
Think of squeezing a balloon.
The balloon changes shape, but it remains intact.
That's similar to what happens with a bulging disc.
Many bulging discs are discovered incidentally during MRI scans performed for unrelated reasons.
In fact, many people have bulging discs and experience no pain whatsoever.
Problems occur when the bulge begins irritating or compressing a nearby spinal nerve.
What Is a Herniated Disc?
A herniated disc occurs when the outer fibers of the disc develop a tear.
Through that opening, some of the soft inner material pushes outward.
This displaced material can irritate nearby nerves, producing symptoms such as:
- Sciatica
- Neck pain
- Arm pain
- Leg pain
- Numbness
- Tingling
- Muscle weakness
Not every herniated disc causes symptoms.
Likewise, not every painful MRI finding requires surgery.
The location of the injury often matters more than its size.
Which One Is Worse?
Patients often assume a herniated disc must always be worse than a bulging disc.
The truth is much more complicated.
A relatively small bulging disc pressing directly on a spinal nerve may produce severe pain.
Meanwhile, a larger herniated disc located away from sensitive nerves may produce very few symptoms.
What really matters is:
✔ Is the disc irritating a spinal nerve?
✔ Does the MRI match your symptoms?
✔ Are neurological changes present?
✔ Is the condition improving?
